A document for registered activity (DRD form) in order to prove that it is registered as legal person to carry out the activity related to the subject-matter of the public procurement contract or a proof that it is affiliated in professional association in accordance with the regulations of the country where it is registered.

To prove his/her personal circumstance, the economic operator shall submit the following documents:

— Statement of the economic operator that, within the last 5 years, he/she has not had any legally effective judgment for participation in a criminal organization, corruption, fraud or money laundering, pronounced against him/her,

— Confirmation that he/she is not subject to a…

… bankruptcy procedure opened by a competent authority,

… liquidation procedure opened by a competent authority,

— Confirmation of paid taxes, contributions and other public dues issued by a competent authority in the country where the economic operator is registered,

— Confirmation issued by the Registry of Penalty for criminal acts legal entities that there is not issued legal minor penalty…

… on participation in procedures for public call, procedure for awarding public contracts and contracts for public private partnership,

… temporary or permanent prohibition for performing certain activity,

— Confirmation that there is no legally effective judgment imposing a misdemeanor sanction involving prohibition to perform a profession, activity or duty, or prohibition to perform a specific activity, imposed against him/her.

Економска и финансиска состојба:

Bank statement no older than 10 days from the deadline for submission of bids.

— balance sheets certified by a competent authority, i.e. audited balance sheet for the last fiscal year or extracts from the balance sheets where publication of the balance sheet is required under the law of the country in which the economic operator is established,

— income statement certified by a competent authority, i.e. audited income statement for the last fiscal year or extracts from the income statement where publication of the income statement is required under the law of the country in which the economic operator is established.

Минимално ниво(а) на стандарди:

The average amount of total assets from the balance sheets of economic operator for the last three fiscal years (2008, 2009 and 2010) not less than 50 million MKD.If the economic operator performs the activity for a period less than three years minimum requirement for determining the economic and financial situation is the economic operator to achieve minimum total annual assets of 50 000 000 MKD for the period for which the data is available.

Техничка и професионална способност:

— Statement of technical equipment and capacity of economic operator to provide same or similar type of services to the subject of the procurement,

— Statement of the engaged technical staff with a list of persons who will be responsible for implementation of the contract,

— Statement that the persons who will be determined by the bidder as team members as well the team manager will be available to realize the subject of procurement throughout the period of duration of the contract. They can be replaced only after prior notification and approval by the contracting authority, but the new proposed persons must meet all conditions required in the technical and professional ability.

Минимално ниво(а) на стандарди:

— At least 3 professionals responsible for implementation of procurement with minimum 3 years experience in the field. As evidence shall be attached list of names of persons who should be employed by the bidder. Regular employment can be confirmed by a photocopy of the form M1/M2,

— The Team Manager should have minimum 5 years working experience in the field. It should be in a regular employment employed by the bidder and that will be confirmed by a photocopy of the form M1/M2,

— Reference list of the bidder for the same / similar services in the past 3 years with value of not less than 12 000 000,00 MKD per particular reference confirmed by the users.

The appeal shall be submitted within eight days as of the day of:

— Publishing the notice for awarding of public procurement contract related to the data, actions or not taking actions of the notice,

— Opening of the tenders related to the actions or not taking actions relating to the tender documentation, i. e opening tender procedure,

— Reception of the decision which decides about single right of the procedure for awarding of public procurement contract related to the determination of the ability of the tenderers for participation or evaluation of the tenders and decision or,

— The awareness about illegal carrying out of the procedure for awarding of public procurement contract within one year of the day of completion of the performed procedure at the latest.

Notice: The appeal must be suvmitted in person or by registered post to the Services Authority and to the State Commission together.
