IV.2.1) In order to prove its personal circumstances, the economic operator should submit the following documents:

— Statement of the economic operator that, within the last five years, he/she has not had any legally effective judgment for participation in a criminal organization, corruption, fraud or money laundering, pronounced against him/her,

— Confirmation that he/she is not subject to a…

… bankruptcy procedure opened by a competent authority,

… liquidation procedure opened by a competent authority,

— Confirmation of paid taxes, contributions and other public dues issued by a competent authority in the country where the economic operator is registered,

— Confirmation from the Registry of Offences for Committed Crimes by Legal…

… Persons that a secondary sentence for prohibition of participation on public bidding procedures or awarding of public procurement or public private partnerships contracts has not been issued against him/her,

… Persons, that a secondary sentence for provisional or permanent prohibition to execute a specific business activity has not been issued against him/her,

— Confirmation that there is no legally effective judgment imposing a misdemeanour sanction involving prohibition to perform a profession, activity or duty, or prohibition to perform a specific activity, imposed against him/her.

IV.2.2) Capability of the economic operator to perform a professional activity:

— Document on registered business activity DRD form.

In order to prove its capability to perform a professional activity, the economic operator should submit a document on registered business activity as proof for being registered as natural or legal person for performing business activity related to the subject matter of the public procurement contract or proof that it belongs to a respective professional association in accordance with the regulations in the country where it is established.

Required documents for proving the economic circumstances of the economic operator:

— Report on the Balance Sheet notarized by the competent authority, or audited Balance Sheet, or excerpts from the Report on the Balance Sheet in cases when the disclosure of the Balance Sheet is prescribed by the law in the country where the economic operator is registered.

Минимално ниво(а) на стандарди:

Minimum criteria for determining the economic and financial circumstances of the economic operator:

— Minimum revenue of 1.500.000 EUR or 92.250.000,00 denars in aggregate for the last three years (2009, 2010, 2011).

Техничка и професионална способност:

Required documents for proving the technical and professional capability of the economic operator:

— Copy of at least 3 (three) contracts for delivery of theatrical curtains in the last 2 years with an individual value of 500 000 EUR or 30 750 000,00 den.,

— Copy of at least 15 (fifteen) contracts for cooperation (various purposes) with theatres or concert halls with a capacity of at least 500 seats,

— At least 18 (eighteen) letters of confirmation, of which 3 for delivery of theatrical curtains in the last 2 years, and 15 for realised cooperation (various purposes) with theatres or concert halls with a capacity of at least 500 seats, from the recipients thereof, or in case such letters may not be furnished due to reasons outside the control of the economic operator, only a statement confirming the realised deliveries, that is cooperation,

— List of experts regularly employed by the Carrier of the procurement, that is, graduated artists, designers or theatrical workers with adequate theatrical education, by submitting the M1/M2 forms as proof of their regular employment with the Carrier of the procurement, their CVs and diplomas. The list of employed experts should include at least 5 (five) experts with proven work experience of at least 10 years as graduated artists, designers or theatrical workers with adequate theatrical education,

— Statement for employment as an artist by indicating his/her name and surname, CV, diploma, proof of min. 10-year work experience and statement by the artist that he/she agrees to decorate the theatrical curtains,

— Samples of all technological curtains of size 2m2 in accordance with the defined materials in the technical specification. Also the preliminary design drawings of the overall motif to be applied on the curtains in the technical specification, items 1 and 2 (free selection of the motif – proposal of the economic operator), should be submitted in a 1:20 scale. Samples of the materials envisaged in the technical specification for items 1 and 2 of size 2m2, including the fragment of the motif for the real size curtains should also be submitted. Additionally, sample of the material envisaged in the technical specification for item 3 of size 2m2, including the embroidery as per the provided drawing (drawing no. 1), appendix to the technical specification, repeated eight times (drawing no. 2). The fragment of the embroidery for the given motif should be in real size,

— Notification by the economic operator which contract elements will be subcontracted.
