— certificate that is not a subject to bankruptcy procedure issued by competent authority,
— certificate that is not a subject to liquidation procedure issued by competent authority,
— certificate for paid taxes, contributions and other public duties by the competent authority in the country where that economic operator is established and,
— certificate that is announced sanction for misdemeanor with an effective court decision, resulting in prohibition for perusing professional activity or duty i.e. temporary prohibition for performing professional activity,
— certificate from the Register of sanctions that the economic operator doesn’t have a prohibition as a result of criminal record, which results with…
… restriction to participate to public bid, bid agreements or in public-private partnerships,
… temporary or continuously restriction to perform professional activity,
— document for registered activities – DRD form, issued by the Central Registry or appropriate document issued by the authority in the country where it is established,
— valid permit/decision/license for conducting the activity-sale of drugs, issued by the Ministry of health of the R. Macedonia or appropriate document issued by the competent authority in the country where it is established.

— Statement regarding the profit and loss account for the last 3 year (2009, 2010 and 2011) issued by a competent authority, or audit report for the last 3 fiscal year (2009, 2010 and 2011) issued by reputable institution.

Positive statement regarding the profit and loss account fot the last 3 year (2009,2010 and 2011) or audit report for the last 3 fiscal year (2009, 2010 and 2011).
Техничка и професионална способност:
— List of main completed deliveries of drugs in the last 3 years, indicating the quantities the amounts, the dates and the recipients with Certificate of the procurement by the applicant issued by the procuring entity if it is not possible to provide by the reason out of control, only with statement for procurement,

Valid approval for releasing in circulation of the final drugs that are subject of procurement, issued by the Ministry of Health of the Republic of Macedonia-Bureau for drugs;

— Authorization for the use the approval for release in circulation of final drug, by the holder of the approval of the offered drugs.
