IV.2.1) In order to prove its personal circumstances, the economic operator should submit the following documents:

- Statement of the economic operator that, within the last five years, he/she has not had any legally effective judgment for participation in a criminal organization, corruption, fraud or money laundering, pronounced against him/her;

- Confirmation that he/she is not subject to a…

… bankruptcy procedure opened by a competent authority;

… liquidation procedure opened by a competent authority;

- Confirmation of paid taxes, contributions and other public dues issued by a competent authority in the country where the economic operator is registered;

- Confirmation from the Registry of Offences for Committed Crimes by Legal…

… Persons that a secondary sentence for prohibition of participation on public bidding procedures or awarding of public procurement or public private partnerships contracts has not been issued against him/her;

… Persons, that a secondary sentence for provisional or permanent prohibition to execute a specific business activity has not been issued against him/her;

- Confirmation that there is no legally effective judgment imposing a misdemeanour sanction involving prohibition to perform a profession, activity or duty, or prohibition to perform a specific activity, imposed against him/her.

- Document on registered business activity DRD form.

In order to prove its capability to perform a professional activity, the economic operator should submit a document on registered business activity as proof for being registered as natural or legal person for performing business activity related to the subject matter of the public procurement contract or proof that it belongs to a respective professional association in accordance with the regulations in the country where established.

Економска и финансиска состојба:

- appropriate bank statements or Bank Certificates;

- Report on the Balance Sheet notarized by the competent authority, that is, audited Balance Sheet, or excerpts from the Report on the Balance Sheet in cases when the disclosure of the Balance Sheet is prescribed by the law in the country where the economic operator is registered

- Statement of the overall turnover of the company (data from the Income Statement issued by the authorised body, that is Revised Income Statement) and where suitable, statement for the turnover in the area subject to the Public Procurement for the maximum of three financial years containing such information depending on the date of establishment of the company or the date of commencement of the operations depending on the availability of the information.

Минимално ниво(а) на стандарди:

- Availability or access to funds amounting amount 120 million denars and

- Minimum revenue of 150.000.000,00 denars individually in the last 3 (three) years (2009, 2010 and 2011).

Техничка и професионална способност:

- Licence A for performing works issued by the Ministry of Transport and Communications.

- List of employed engineers and enclosed Certificate from the Employment Agency of the Republic of Macedonia and copies of their diplomas or certificates on completed education.

- Statement on dedicated two engineers for each phase of the works subject to the procurement to control the quality of the executed activities.

- List of employed technically and professionally qualified personnel in the area of technical sciences who shall be included in the realisation of the procurement accompanied by a Certificate from the Employment Agency of the Republic of Macedonia and copies of their diplomas or certificates on completed education.

- Copy of contracts for executed mounting works (thermo-technical installations or electronic installations) accompanied by a Confirmation of duly executed high-quality services issued by the Contracting authority describing the type, subject, value and time of executed services.

- Copy of Property Deed or Contract for leasing storage facility or any other real estate property intended for storage of materials and execution of preparatory works.

- Notification on the contracts elements that the economic operator intends to subcontract and information on the subcontractor.

Минимално ниво(а) на стандарди:

- A License Contractor;

- Economic operator shall have at least 15 engineers employed on full-time basis with the carrier of the procurement who shall be included in the implementation of the procurement;

- Economic operator shall have dedicated two engineers for each phase of the works subject to the procurement to control the quality of the executed activities;

- Economic operator shall have at least 70 employed technically and professionally qualified personnel in the area of technical sciences who shall be included in the realisation of the procurement;

- Reference list of at least 5 mounting completed works (thermo-technical installations or electronic installations) in facilities exceeding 8,000m2 in the last five years;

- Reference list of at least 5 construction and craftsmanship works in facilities exceeding 8,000m2 in the last five years;

- Economic operator shall have available storage facility or any other real estate property intended for storage of materials and execution of preparatory works;
