Capability of the economic operator to perform a professional activity: — Document on registered business activity DRD form. In order to prove its capability to perform a professional activity, the economic operator should submit a document on registered business activity as proof for being registered as natural or legal person for performing business activity related to the subject matter of the public procurement contract or proof that it belongs to a respective professional association in accordance with the regulations in the country where it is established. Standards for quality systems: — ISO 9001 — Written statement that it will submit attest certificates for the quality of used materials and equipment in compliance with the prescribed standards; — Statement that in case it is awarded public procurement contract it will contract adequate technical body for additional quality control and assessment concerning the used materials and performed works. Standards for environmental management: — ISO 14001. Performance guarantee: Yes [10,00%].

In order to prove its personal circumstances, the economic operator should submit the following documents:

— Statement of the economic operator that, within the last five years, he/she has not had any legally effective judgment for participation in a criminal organization, corruption, fraud or money laundering, pronounced against him/her;

— Confirmation that he/she is not subject to a…

— Confirmation of paid taxes, contributions and other public dues issued by a competent authority in the country where the economic operator is registered;

… Persons that a secondary sentence for prohibition of participation on public bidding procedures or awarding of public procurement or public private partnerships contracts has not been issued against him/her;

… Persons, that a secondary sentence for provisional or permanent prohibition to execute a specific business activity has not been issued against him/her;

— Confirmation that there is no legally effective judgment imposing a misdemeanour sanction involving prohibition to perform a profession, activity or duty, or prohibition to perform a specific activity, imposed against him/her.

Економска и финансиска состојба:

— Report on the Balance Sheet notarized by the competent authority, that is, audited Balance Sheet, or excerpts from the Report on the Balance Sheet in cases when the disclosure of the Balance Sheet is prescribed by the law in the country where the economic operator is registered.

Минимално ниво(а) на стандарди:

— Minimum revenue of 150.000.000,00 denars individually in the last 3 years (2009, 2010 and 2011).

Техничка и професионална способност:

— Copy of at least 3 contracts for delivery of interior finishing in the last 5 years with an individual value of 20.000.000,00 denars;

— Copy of at least 6 contracts for performing construction and craftsmanship works on buildings with an individual area not lower than 3 000 m²;

— At least 9 letters of confirmation for duly and timely completion of works, of which 3 letters of confirmation for interior finishing in the last three years and 6 (six) letters of confirmation for completed construction and craftsmanship works;

— A License Contractor;

— M1/M2 forms as proof for regular employment of at least 100 persons, of which submit for three persons A authorisation for contracting engineer and for one person B authorisation for contracting engineer;

— List of responsible persons (name, surname and CV) who will be directly involved in the realisation of each stage of the procurement subject;

— Reference list of completed works in the last five years, including values, dates, clients;

— Statement on technical equipment and capacities of the economic operator, including description of the equipment that will be directly used in the procurement realisation;

— Photographs of completed construction and craftsmanship works and interior finishing;

— Notification on the contract elements that the economic operator intends to give to subcontractors including their data, whereas the subcontracted part must not exceed 40 % of the total work volume. In case the economic operator does not intend to engage subcontractors it shall furnish a statement that it will perform all works on its own.

Минимално ниво(а) на стандарди:

— To have completed at least 3 contracts for interior finishing in the last 5 years with an individual value of 20.000.000,00 denars;

— To have completed at least 6construction and craftsmanship works on buildings with an individual area not lower than 3 000 m²;

— The economic operator must have a Contractor's A License in compliance with the Construction Law of the Republic of Macedonia;

— Proof of at least 100 persons employed, of whom three persons with A authorisation for contracting engineer and one person with B authorisation for contracting engineer.
