In order to prove its personal status, the economic operator should submit the following documents:

— statement of the economic operator that no effective verdict has been issued against the economic operator within the past 5 years for participation in a crime organization, corruption, fraud or money laundering;

— confirmation stating that no bankruptcy procedure has been initiated by a competent body;

— confirmation stating that no liquidation procedure has been initiated by a competent body;

— certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered;

— confirmation from the Register of penalties for committed criminal offenses of legal entities proving that no secondary penalty -…

… ban on participating in open call procedures, awarding public procurement contracts and contracts on public - private partnership has been pronounced;

… temporary or permanent ban on performing particular activity has been pronounced;

— confirmation stating that no misdemeanour sanction - ban on the performance of the profession, activity or duty, i.e. temporary ban on the performance of a particular activity has been pronounced with an effective verdict.

Capability of the economic operator for performing professional activity:

— Document for registered activity.

Економска и финансиска состојба:

Necessary documents for proving the economic status of the economic operator:

— balance sheet verified by a competent body, i.e. an audited balance sheet, or extracts from the balance sheet, in cases when the disclosure of the balance sheet is regulated by law in the country where the economic operator is registered; or

— extract from the overall turnover of the enterprise (datum from the profit and loss account issued by a competent body, i.e. an audited profit and loss account) and, where appropriate, from the turnover in the area covered by the public procurement contract and for maximum of the last three financial years for which such information is available depending on the date when the enterprise was established or commenced with operation and depending on the availability of such information.

Минимално ниво(а) на стандарди:

Minimum criteria for determining the economic and financial status of the economic operator:

— positive financial operation in the last 3 (three) business years (2010, 2011 and 2012);

— minimum 60 000 000 MKD (sixty million Denars) realized revenue for each of the last 3 (three) business years in particular (2010, 2011 and 2012).

Техничка и професионална способност:

Necessary documents for proving the technical or professional capability of the economic operator:

— Licence 'B' for contractor, for the construction of second category buildings, in accordance with the Law on Construction (Official Gazette of RM no. 130/09, 124/10, 18/11, 36/11, 54/11, 13/12, 144/12 and 25/13);

— Statement from the economic operator that it possesses storage house and warehouse, necessary for realization of the procurement, by indicating the location thereof.

— List of the equipment and Statement of the economic operator that it possesses the necessary technical equipment as follows: 2 (two) vibro tiles, 2 (two) vibro rollers, 3 (three) loading machines, 1 (one) CNC machine, 2 (two) impregnation lines, 2 (two) sander machines and 5 (five) stone-cutting machines, by enclosing appropriate evidence for possessing thereof (traffic licence, leasing contract, inventory list, purchasing invoice, purchasing contract, etc.);

— Evidence for educational and professional qualifications of the economic operator (submission of a copy of M1/M2 Forms for minimum 5 (five) employees, as well as a copy of the required authorizations issued by the Chamber of Authorized Architects and Engineers).

Minimum criteria for determining the technical and professional status of the economic operator:

Licence for construction of second category buildings, in accordance with the Law on Construction (Official Gazette of RM no. 130/09, 124/10, 18/11, 36/11, 54/11, 13/12, 144/12 and 25/13);

— The economic operator should possess storage house for construction materials and warehouse.

— The economic operator should possess the following equipment for realization of the respective procurement as follows:

- 2 (two) vibro tiles;

- 2 (two) vibro rollers;

- 3 (three) loading machines;

- 1 (one) CNC machine;

- 2 (two) impregnation lines;

- 2 (two) sander machines;

- 5 (five) stone-cutting machines.

— The economic operator should have employed minimum 5 (five) persons, as follows:

- minimum 1 (one) mechanical engineer with B authorization for construction, issued by the Chamber of Authorized Architects and Engineers.

- minimum 2 (two) construction engineers with B authorization for construction, issued by the Chamber of Authorized Architects and Engineers.

- minimum 1 (one) engineer - architect with B authorization for construction, issued by the Chamber of Authorized Architects and Engineers.

- minimum 1 (one) electrical engineer with B authorization for construction, issued by the Chamber of Authorized Architects and Engineers.
