In order to prove its personal status, the economic operator should submit the following documents:

— statement of the economic operator that no effective verdict has been pronounced against the economic operator within the past 5 years for participation in crime organization, corruption, fraud or money laundering;

— certificate stating that no bankruptcy procedure has been initiated by a competent body;

— certificate stating that no liquidation procedure has been initiated by a competent body;

— certificate of paid taxes, benefits and other public duties, issued by the competent body in the country where the economic operator is registered;

— certificate from the Register of penalties for committed acts of crime by legal entities stating that no secondary penalty -…

… ban on participating in open call procedures, procedures for awarding public procurement contracts and public-private partnership contracts has been issued to the economic operator;

… temporary or permanent ban on performing a particular activity has been issued to the economic operator;

— certificate stating that an effective verdict has not been issued to the economic operator imposing misdemeanour sanction – banning the economic operator from conducting a business, activity or duty, i.e. a temporary ban on performing a particular activity. IV.2.2) Capability of the economic operator for performing professional activity:

— Document for registered activity DRA - Form.

Економска и финансиска состојба:

— balance sheet verified by a competent body, i.e. an audited balance sheet, or extracts from the balance sheet, in cases where the disclosure of the balance sheet is regulated by law in the country where the economic operator is registered;

— extract from the overall turnover of the enterprise (datum from the profit and loss account issued by a competent body, i.e. an audited profit and loss account), maximum for the last three financial years for which such information is available depending on the date when the enterprise was established or commenced with operation, and depending on the availability of such information.

Минимално ниво(а) на стандарди:

— positive financial operation in the last 3 (three) business years (2010, 2011 and 2012).

— minimum 80,000,000.00 MKD (eighty million Denars) realized revenue for each of the last 3 business years in particular (2010, 2011 and 2012).

Техничка и професионална способност:

Necessary documents for proving the technical or professional capability of the economic operator:

— B Licence for contractor, for performing second category constructions, in accordance with the Construction Law (Official Gazette of RM no. 130/09,124/10, 18/11, 36/11, 54/11, 13/12, 144/12 and 25/13);

— Statement from the economic operator stating that it possesses storehouse and warehouse premise, necessary for realization of the procurement by specifying the locations thereof.

— List of the equipment and statement for possessing the necessary technical equipment as follows: 5 vibro tiles, 4 vibro rollers, 5 loading machines, 1 (one) surface-rough machine, 1 CNC machine, 3 impregnation lines, 2 calibration machines, 3 sander machines, 8 stone-cutting machines, by enclosing an evidence as follows: traffic license, leasing contract, inventory list, purchase and sale invoice, etc.

— Evidence for educational and professional qualifications of the economic operator (submission of a copy of M1 and M2 forms for all of the 55 employees, as well as a copy of the required authorizations issued by the Chamber of Authorized Architects and Engineers.

Минимално ниво(а) на стандарди:

— Licence for performing second category constructions, in accordance with the Construction Law (Official Gazette of RM no. 130/09,124/10, 18/11, 36/11, 54/11, 13/12, 144/12 and 25/13);

— The economic operator should possess storehouse for the construction materials as well as a warehouse premise.

— The economic operator should possess the following equipment for realization of the respective procurement as follows:

— 5 vibro tiles;

— 4 vibro rollers;

— 5 loading machines;

— 1 surface-rough machine;

— 1 CNC machine;

— 3 impregnation lines;

— 2 calibration machines;

— 3 sander machines;

— 8 stone-cutting machines.

— The economic operator should have minimum 55 employees as follows:

— minimum 1 mechanical engineer with B authorization for construction, issued by the Chamber of Authorized Architects and Engineers.

— minimum 2 construction engineers with B authorization for construction, issued by the Chamber of Authorized Architects and Engineers.

— minimum 1 Bachelor of Engineering - Architect with B authorization for construction, issued by the Chamber of Authorized Architects and Engineers.

— minimum 1 electrical engineer with B authorization for construction, issued by the Chamber of Authorized Architects and Engineers.

— minimum 50 workers.
