In order to prove its personal status, the economic operator should submit the following documents:

— statement of the economic operator that no effective verdict has been issued against the economic operator within the past five years for participation in a crime organization, corruption, fraud or money laundering;

— confirmation stating that no bankruptcy procedure has been initiated by a competent body;

— confirmation stating that no liquidation procedure has been initiated by a competent body;

— certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered;

— confirmation from the Register of penalties for committed criminal offenses of legal entities proving that no secondary penalty -…

… ban on participating in open call procedures, awarding public procurement contracts and contracts on public - private partnership has been pronounced;

… temporary or permanent ban on performing particular activity has been pronounced;

— confirmation stating that no misdemeanour sanction - ban on the performance of the profession, activity or duty, i.e. temporary ban on the performance of a particular activity has been pronounced with an effective verdict.

Ability for performing professional activity of the economic operator:

— document for registered activity.

In order to prove the ability for performing professional activity, the economic operator should submit a document for registered activity (DRA form), as a proof that it has been registered as a physical person or legal entity for performing the activity related to the subject of the public procurement contract.

Економска и финансиска состојба:

— balance sheet verified by a competent body, i.e. an audited balance sheet, or extracts from the balance sheet, in cases where the disclosure of the balance sheet is regulated by law in the country where the economic operator is registered;

— extract from the overall turnover of the enterprise (datum from the profit and loss account issued by a competent body, i.e. an audited profit and loss account) and, where appropriate, from the turnover in the area covered by the public procurement contract and for maximum of the last three financial years for which such information is available depending on the date when the enterprise was established or commenced operation and depending on the availability of such information.

Минимално ниво(а) на стандарди:

— positive financial operation in the last three financial years (2009, 2010 and 2011);

— the realization for the last three financial years (cumulatively) - total revenue of minimum 90,000,000.00 EUR in MKD equivalence.

Техничка и професионална способност:

— Licence “B” for contractor, for the construction of second category buildings, in accordance with the Law on Construction (Official Gazette no. 51/05, 137/07, 91/09, 124/10, 18/11, 53/11, 13/12 and 144/12), issued by the Ministry of Transport and Communications of RM i.e. in the event of foreign bidder, a Confirmation is to be submitted for construction of second category buildings, issued by the appropriate body of the state administration competent for performance of the works for spatial arrangement of RM, pursuant to Article 42 of the Law on Construction.

— List of engaged qualified staff for the realization of the respective procurement by submitting proof as an enclosure, as follows:

— M1/M2 forms, i.e. in the event of foreign bidder an appropriate valid proof is to be submitted proving that the persons are employed at the bidder.

— Authorizations issued by the Chamber of Certified Architects and Certified Engineers of RM, i.e. in the event of foreign bidder, a Confirmation for the credibility of the authorization of the foreign architect and engineer for construction is to be submitted, issued by the Chamber of Certified Architects and Certified Engineers of RM, pursuant to Article 42 and Article 104 of the Law on Construction.

— CV for the responsible structural engineers, enclosing a proof for the necessary working experience for successful management of at least 1 (one) project for the construction of traffic tunnel, issued by the company that hired the person.

— List of the available equipment for the performance of the subject of procurement, by submitting:

— proof for ownership - inventory list with contract on purchase and sale, etc. (if owned by the bidder);

— leasing contract (if the equipment is gained through leasing).

Минимално ниво(а) на стандарди:

— Licence for constructing second category buildings, in accordance with the Law on Construction; (Official Gazette no. 51/05, 137/07, 91/09, 124/10, 18/11, 53/11, 13/12 and 144/12);

— Qualified and technical staff for the realization of the respective procurement, composed of at least:

— 2 persons in the capacity of responsible structural engineers with Authorisation “B” for construction from the respective area, with working experience in managing at least 1 (one) project for the construction of traffic tunnel;

— 1 electrical engineer with Authorization - B category for construction in the respective area;

— 1 mechanical engineer with Authorization - B category for construction in the respective area;

— 1 traffic engineer with Authorization - B category for construction in the respective area;

— 1 architect with Authorization - B category for construction in the respective area;

— 1 geodetic engineer with Authorization - B category for construction in the respective area; and

— 50 common workers (technical staff).

— Appropriate technical equipment for the performance of the construction works, subject of procurement (which must not be older than 4 years), as follows:

— minimum 4 machines for poles or wells, with dimension of minimum f600 to f1200, out of which minimum 2 machines have dimension of f1200;

— minimum 2 self-propelled cranes with cargo capacity of minimum 20t;

— minimum 2 shotcreting machines;

— minimum 2 machines for overstraining concrete carriers; and

— minimum 2 cement injecting machines.
