Information and formalities necessary for evaluating if the requirements are met: The contracting authority shall exclude from the procedure for awarding a public procurement contract, any economic operator that:

— Within the last 5 years has had a legally effective judgment pronounced against him/her for participation in a criminal organization, corruption, fraud or money laundering,

— Is subject to a bankruptcy procedure or a liquidation procedure,

— Has unpaid taxes, contributions or other public dues,

— That has a misdemeanor sanction involving prohibition to perform a profession, activity or duty, or prohibition to perform a specific activity, imposed against him/her and,

— That gives false information or fails to submit the data requested by the contracting authority to prove his/her personal circumstance, the economic operator shall submit the following documents:

—— Statement of the economic operator that, within the last five years, he/she has not had any legally effective judgment for participation in a criminal organization, corruption, fraud or money laundering, pronounced against him/her,

—— Confirmation that he/she is not subject to a…

… bankruptcy procedure initiated by a competent authority,

… liquidation procedure opened by a competent authority,

—— Confirmation of paid taxes, contributions and other public dues issued by a competent authority in the country where the economic operator is registered,

—— Confirmation that there is no legally effective judgment imposing a misdemeanor sanction involving prohibition to perform a profession, activity or duty, or prohibition to perform a specific activity, imposed against him/her.

To prove the capability to perform a professional activity, the economic operator will have to submit a document for a registered activity as a proof that the operator is registered as a natural person or a legal entity to perform an activity related to the public procurement contract or proof of membership in an appropriate professional association in accordance with the regulations of the country where the operator is registered.

Information and formalities necessary for evaluating if the requirements are met: — Report on the balance sheet notarized by a competent authority, i.e. audited balance sheet, or excerpts from the balance sheet in cases when the disclosure of the balance sheet is prescribed by law in the country where the economic operator is registered,

— Statement of the total turnover of the enterprise (information from the income statement issued by a competent authority, i.e. audited income statement) and statement of the turnover from the sale of bitumen.

Minimum level(s) of standards possibly required: To realize turnover from sale of bitumen of at least 20 000 000 MKD in the last year - 2011.

Техничка и професионална способност:

Information and formalities necessary for evaluating if the requirements are met:

— List of major deliveries made by the bidders or candidates, with values, dates, buyers (contracting authorities or economic operators), together with certificates for the performed deliveries issued by the recipients or, if such certificates cannot be provided due to reasons beyond the control of the economic operator only with the operator's statement of the performed deliveries,

— Description of the technical equipment and capacity of the economic operator (vehicles for which they will submit traffic identity cards, wherefrom the data about the vehicles' technical characteristics shall be obtained),

— Statement about the engaged technical staff (list of the drivers employed at the economic operator),

— Certificate for the quality of the bitumen with characteristics, issued by the producer,

— Certificate – Attests for the quality of the bitumen issued by the authorized institution in the Republic of Macedonia, confirming the standard MKS U.M3.010.

Minimum level(s) of standards possibly required:

— To posses at least three (3) vehicles for transport of bitumen,

— To possess drivers of the vehicles for transport of bitumen.
