A document for registered activity (DRD form) in order to prove that it is registered as legal person to carry out the activity related to the subject-matter of the public procurement contract or a proof that it is affiliated in professional association in accordance with the regulations of the country where it is registered.

To prove his/her personal circumstance, the economic operator shall submit the following documents:

— Statement of the economic operator that, within the last five years, he/she has not had any legally effective judgment for participation in a criminal organization, corruption, fraud or money laundering, pronounced against him/her;

— Confirmation that he/she is not subject to a…

… bankruptcy procedure opened by a competent authority;

… liquidation procedure opened by a competent authority;

— Confirmation of paid taxes, contributions and other public dues issued by a competent authority in the country where the economic operator is registered;

— Confirmation issued by the Registry of Penalty for criminal acts legal entities that there is not issued legal minor penalty…

… on participation in procedures for public call, procedure for awarding public contracts and contracts for public private partnership;

… temporary or permanent prohibition for performing certain activity;

— Confirmation that there is no legally effective judgment imposing a misdemeanor sanction involving prohibition to perform a profession, activity or duty, or prohibition to perform a specific activity, imposed against him/her.

Економска и финансиска состојба:

Bank statement no older than 10 days from the deadline for submission of bids:

— balance sheets certified by a competent authority,i.e. audited balance sheet for the last fiscal year or extracts from the balance sheets where publication of the balance sheet is required under the law of the country in which the economic operator is established;

— income statement certified by a competent authority, i.e. audited income statement for the last fiscal year or extracts from the income statement where publication of the income statement is required under the law of the country in which the economic operator is established.

Минимално ниво(а) на стандарди:

The average amount of total income balance condition of economic operator for the last three fiscal

years (2010, 2011 and 2012) not less than 30 million MKD.If the economic operator performs the activity for a period less than three years minimum requirement for determining the economic and financial situation is the economic operator to achieve minimum total annual assets of 30 million MKD for the period for which the data is available.

Техничка и професионална способност:

— Statement of technical equipment and capacity of economic operator to provide same or similar type of services to the subject of the procurement,

— Statement of the engaged technical staff with a list of persons who will be responsible for implementation of the contract,

— Statement that the persons who will be determined bythe bidder as team members as well the team manager will be available to realize the subject of procurement throughout the period of duration of the contract.

They can be replaced only after prior notification and approval by the contracting authority, but the new proposed persons must meet all conditions required in the technical and professional ability,

— Statement that the persons who will be determined by the bidder as team members as well the team manager will be available to realize the subject of procurement throughout the period of duration of the contract.

Минимално ниво(а) на стандарди:

— At least 3 professionals responsible for implementation of procurement with minimum 2 years experience in the field. As evidence shall be attached list of names of persons who should be employed by he bidder. Regular employment can be confirmed by a photocopy of the form M1/M2.

— The Team Manager should have minimum 5 years working experience in the field. It should be in a regular employment employed by the bidder and that will be confirmed by a photocopy of the form M1/M2,

— Reference list of the bidder for the same / similar services in the past three years with value of not less than 12.000.000,00 MKD per particular referenceconfirmed by the users.

Letters of cooperation from at least 3 national print (which is distributing the territory) and 4 national broadcasters with national coverage, which will indicate that the disposal of the economic operator to implement the procurement.

Потребни депозити и гаранции: 3 % gyarantee of the participation of the total value of the offer without VAT.

10 % bank garantee of the value of the contract for quality performing of the contract.
