— Document (Approval) that the economic operator, i.e. its representative offices are registered for performing the activity stated in the tender, and it can perform whole-sales activities regarding medical devices as prescribed with the Law on Medicinal Products and Medical Devices, issued by the Ministry of Health in RM-Drugs Bureau or other valid document issued by an authority in the country where the tenderer is registered;

— A Valid Decision with all attachments completed, that the bags are approved to be released into circulation on the territory of the Republic of Macedonia in compliance with the Law on Medicinal Products and Medical Devices;
— A catalogue in which the subject of this public procurement will be clearly stated, and where the requirements from the technical specification can be determined;
— Samples from the required subject stated in this public procurement;
— A Statement given by the economic operator that drugs delivered will be with expiry date at least 12 months from the day of bags delivery, signed and sealed by the bidder;
— Statement from the manufacturer or a certificate signed and sealed by the manufacturer that…
