Information and formalities necessary for evaluating if the requirements are met: The contracting authority shall exclude any economic operator from the contract award procedure, provided that it:

— is a under a bankruptcy or liquidation procedure;

— has unpaid taxes, contributions or other public duties, unless the economic operator is approved delayed payment of taxes, contributions or other public duties, in accordance with the special regulations and pays them on regular basis;

— has been imposed a secondary sentence prohibition on participation in open call procedures, awarding public procurement contracts and contracts for public private partnership;

— has been imposed a secondary sentence temporary or permanent prohibition on performing a certain activity;

— is being pronounced a misdemeanor sanction prohibition on practising profession, performing activity or duty, i.e. temporary prohibition for performing a particular activity, or

— presents false information or does not submit the information required by the contracting authority.

The economic operator shall submit the following documents for the purpose of proving its personal standing:

— a statement by the economic operator that it has not been imposed a legally valid court decision for participation in criminal organization, corruption, fraud or money laundering in the last 5 years;

— certificate by a competent authority that a bankruptcy procedure has not been initiated;

EN Standard form 02 — Contract notice 7/18:

— certificate by a competent authority that a liquidation procedure has not been initiated;

— certificate by a competent authority from the country where that economic operator is registered for paid taxes, contributions and other public duties;

— certificate by the Register of sentences for committed crimes of legal entities that a secondary sentence…

… prohibition on participation in open call procedures, awarding public procurement contracts and contracts for public private partnership, has not been imposed,

… temporary or permanent prohibition on performing a certain activity, has not been imposed

— certificate that a misdemeanor sanction resulting in prohibition for practising profession, performing activity or duty, i.e. temporary prohibition for performing a particular activity has not been imposed by a legally valid court decision.

Економска и финансиска состојба:

— Balance sheets certified by a competent authority, i.e. audited balance sheets or extracts from the balance sheets, when the announcement of the balance sheet is prescribed by law in the country wherein the economic operator is registered;

— Statement regarding the overall turnover of the enterprise (data on the profit and loss account issued by a competent authority, i.e. audited profit and loss account) for the last 2 financial years.

*Foreign tenderer must submit positive audit report issued by foreign renowned institution registered for performing the audit. The audit report shall contain data on the economic performance of the Foreign tenderer for the last 2 years and should be obligatory translated into Macedonian.

Минимално ниво(а) на стандарди: Average total revenue over 15 000 000 MKD in the last 2 years.
Техничка и професионална способност:

A list of the principal services as Supervisior of the tenderers or the candidates conducted in the past three years containing values, dates, buyers (contracting authorities or economic operators), by providing a statement for the conducted services issued by the recipients, or if such statements cannot be provided due to reasons beyond the control of the economic operator, only with its statement regarding the conducted services.

A statement by the economic operator on the average annual number of employees in the past 3 years.

A statement on the professional qualifications of the economic operator or of its expert staff, and especially of the persons responsible for providing the respective service.

A statement on hired technical staff and technical authorities, regardless of whether they directly belong to the economic operator.

A statement by the economic operator that the specified Supervision team shall be available throughout the duration of the contract.

Quality assurance standards: ISO 9001:2008

Each economic operator shall deliver a document for registered activity in order to prove it is registered as a natural person or legal entity to pursue the activity related to the subject matter of the public procurement contract or a proof that it belongs to a professional association in accordance with the regulations of the country where registered.

Each economic operator shall deliver Licence A for Supervision of category one works in accordance with the Law on Construction.

Минимално ниво(а) на стандарди:

The economic operator to have a minimum of 4 contracts as supervisor in subject area — for services such supervision objects I (a first) category in the last 3 years, with a minimum of 4 applied contracts and/or confirm — recommendations for services rendered as supervision of facilities I (a first) category, issued by the recipients of services.

The economic operator to have a minimum 3 contracts as a supervisor in subject area — for services such as supervision of facilities I category, with a minimum value of each contract 1 200 000 MKD VAT.

The economic operator has an average number of technical staff employed in full time employment in the last year of at least 15 people, a regular employed should be 3 engineers Authorization 'A' for supervision of construction of buildings.

A) Key personnel:

A.1 Position: Chief Supervision/Project Manager must meet the following minimum requirements:

— Professional training: Bachelor of Civil Engineering;

— Work experience at least 10 years experience as supervisor in construction of similar facilities (roads, streets, water and sewer infrastructure);

— Authorization 'A' for the supervision of facilities in the field of construction issued by the competent authority.

A.2 Position: Assistant supervision of hydro-technical works, should meet the following minimum requirements:

— Work experience in construction at least 5 years as supervisor of construction objects hydro-technical direction;

— Authorization 'A' for the supervision of objects from area of construction issued by the competent authority.

A.3 Position: Assistant supervision of electrical work must meet the following minimum requirements:

— Professional training: graduate electrical engineer;

— Work experience in construction at least 5 years;

— Authorization 'A' for the supervision of objects issued by the competent authority.

B) Support staff:

B.1 Position: Surveyor, should meet the following requirements:

— Graduate engineer surveyor with authorization 'A' for the engineer to construction the field geodesy;

— have at least 7 years experience in construction activity.

B.2 Position: Technical laboratory for materials should meet the following requirements:

— At least 7 years experience as laboratory facilities of construction of the same or related field.

* Please attach a Curriculum Vitae (CV) for each of said technical staff on a construction site — requirement Authorization and photocopy of diplomas of completed studies.

Availability of the specified technical staff on a construction site for the whole duration of the contract regardless of whether it belongs to the economic operator, and if said technical staff on site does not belong to the economic operator to prove its availability;

Quality assurance standards: ISO 9001:2008.

Each economic operator shall deliver Licence A for Supervision of category one works in accordance with the Law on Construction.

Потребни депозити и гаранции:

Tender guarantee in the form of bank guarantee (3 %).

Guarantee for implementation of the contract in a quality manner in the form of a bank guarantee (10 %).

All other information about the guarantees are determined in the tender documentation.

Contracting authority will not require from the group of economic operators to associate in a specific legal form, for the purpose of executing the contract.
