IV.2.1) To certify its personal situation, the economic operator must submit the following documents:

— statement of the economic operator that in the last five years it has not been criminally charged for participated in criminal organization, corruption, fraud or money laundry;

— confirmation that there are no bankruptcy proceedings by a competent authority;

— confirmation that there are no proceedings for liquidation by a competent authority;

— confirmation for paid taxes, contributions and other public taxes by competent authority in the country where the economic operator is registered;

— confirmation from the Register of penalties for crimes committed by legal persons that it has not received a minor…

… ban on participation in public bidding procedures, awarding of contracts for public procurement and contracts for public-private partnership;

… penalty: temporary or permanent prohibition for execution of certain activity;

— confirmation that the operator has not received a final court decision: sanction — ban on performing of profession, activity or duty, i.e. temporary prohibition to perform certain activities.

IV.2.2) Ability to perform professional activity by the economic operator:

— Document for registered activity.

2.3 Ability to execute professional activity.

2.3.1 In order to proof the ability to perform professional activity, the economic operator must submit a document for registered activity as proof that it has been registered as legal or physical entity for performing the activity related to the subject of the contract for public procurement or evidence that it belongs to an appropriate professional association in accordance with the regulations of the country where it is registered.

Економска и финансиска состојба:

— report for the balance and data for the success balance issued by a competent authority, i.e. audited balance sheet, or extracts from the balance sheets where publication of the balance sheet is required under the law of the country in which the economic operator is established.

Минимално ниво(а) на стандарди:

Total turnover of financial means for last 3 years over 3 600 000 since 2011 until the day of the opening of bids.

Техничка и професионална способност:

— Copies of at least three signed and executed contracts for the purchases made in the last 3 years of the same or similar type of which one with a value of at least 1 200 000 MKD as well as confirmations for the timely and qualitatively executed procurement and delivery, appropriate to the contracts;

— document that confirms the number of employees issued by competent authority.

Минимално ниво(а) на стандарди:

— At least 3 contracts for procurement of goods, of which one with total value 1 200 000 MKD since 2011 until the day of the opening of the bids;

— at least 3 employed persons.
