— Statement by the economic operator than it has not been sentenced, in the last 5 years, with effective court decision for participation in criminal organization, corruption, fraud or money laundering;

— Certificate that it is not subject to bankruptcy procedure issued by competent authority;

— Certificate that it is not subject to liquidation procedure issued by competent authority;

— Certificate for paid taxes, contributions and other public duties by the competent authority in the country where that economic operator is established;

— Certificate from the Legal Entities Penal Registry that it is not being imposed a side sanction…

… — prohibition to participate in public competitions, contract award procedures and PPP award procedures;

… — temporary or permanent prohibition to perform certain professional activity;

— Certificate that it is not being sanctioned for misdemeanor with an effective court decision, resulting in prohibition for pursuing professional activity or duty, i.e. temporary prohibition for performing professional activity;

To prove its ability for performing professional activity the economic operator must submit:

— Document for registered activity;

— Authorization for wholesale of medical devices issued by the Agency of Medicines, Ministry of Health of the Republic Macedonia.

List of main deliveries from the bidders or of the product candidates completed during the last 3 years with values, dates, buyers info (contracting authorities or economic operators), confirmation for completed service deliveries issued by recipients, or if such a confirmation can not be provided for reasons beyond the control of the economic operator, then its statement for completed deliveries needs to be submitted;

2. Mandatory submission of samples, submitted samples will be tried by the Commission for expert opinion in order to determine whether they are appropriate to the needs of our Clinic; the contracting authority has no obligation to pay or return these;

3. Decision of the Ministry of Health of Republic of Macedonia that the products that are subject of the delivery are registered on the territory of the Republic of Macedonia or a confirmation that the registration is in process, which will be completed by the time the contract is signed;

4. Certificates for quality control issued by competent authorities with recognized competence, that test the conformity of the products that is clearly identified by references to specifications and standards, as individual proof for each requirement as stated in point four in part 2.4.1. of the tender documentation.

Минимално ниво(а) на стандарди:

1. Completed deliveries of the object of procurement;

2. To have adequate quality as requested or stated in the technical specification for the procurement items;

3. The offered medical devices to be registered on the territory of the Republic Macedonia in accordance with the Law of drugs and medical devices;

4. For the lots with number 167-174, the offered medical devices to be manufactured in accordance with the quality systems: ISO 9001 or equivalent, Directive 93/42 EEC or equivalent for medical products and EN 455 or equivalent; for the lots with number.: 169, 170, 171, 172, 173, 174 the offered products to be manufactured in accordance with the quality systems: ISO 10282 or equivalent, Directive 93/42 EEC or equivalent for medical products and EN 455 or equivalent.
