In order to prove its personal condition the economic operator is obliged to submit the following documents:
— Statement of the economic operator that in the past 5 years it was not rendered a final judgement for participation in criminal organization, corruption, fraud or money laundering.
— Confirmation by a competent organ that a…
… bankruptcy procedure was not opened.
… liquidation procedure was not opened.
— Confirmation for paid taxes, contributions and other public duties by a competent organ in the country where the economic operator was registered.
-— Certificate from the Registry of penalties for crimes committed by legal persons that minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him.
— Certificate from the Registry of penalties for crimes committed by legal persons that a minor penalty for temporary or permanent prohibition of a specific activity has not been imposed over him.
— Confirmation that a final judgment — prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.
— Document for registered activity.

Техничка и професионална способност:
Necessary documents to prove the technical or professional capacity of the economic operator:
1. Realized public procurement contracts of the same or similar type.
2. Professional technical and educated by the equipment producer personnel in charge of the installing the equipment (Urea Breath Test System).
3. Professional technical and educated by the equipment producer personnel in charge of maintaining the equipment with technical assistance.
4. Professional technical and educated by the equipment producer personnel for the training of the personnel that will work with the equipment.
5. Technical capacity and equipment of the economic operator for regular servicing of the equipment in the guarantee period.
6. Guarantee of the equipment of 2 (two) years, a period in which the economic operator is obliged to provide free servicing and free replacement of spare parts.
Минимално ниво(а) на стандарди:
Minimum criteria for determining the technical or professional of the economic operator:
1. Evidence for realized public procurement agreements of same or similar type (minimum 1 confirmation from an end user for realized agreement).
2. Reference list for procurement of same or similar type (minimum one reference).
3. Statement and enclosed certificate issued by the producer of the equipment for secured professional technical and educated by the equipment producer personnel in charge of the installing the equipment.
4. Statement and enclosed certificate issued by the producer of the equipment for secured professional technical and educated by the equipment producer personnel in charge of maintaining the equipment with technical assistance.
5. Statement and enclosed certificate issued by the producer of the equipment for secured professional technical and educated by the equipment producer personnel in charge of training of the personnel that will work with the equipment.
6. Statement that it would provide guarantee for the equipment for a period of 2 (two) years, in which period the economic operator is obliged to provide free servicing and free replacement of spare parts.
7. Statement of the economic operator that is technically equipped and with relevant personnel for regular servicing of the equipment in the guarantee period.
