— a statement that no verdict in force has been passed to the economic operator in the last 5 years for participation in a criminal organization, corruption, fraud or money laundry;

— certificate that no bankruptcy proceedings have been instituted by a competent body;

— certificate that no liquidation proceedings have been instituted by a competent body;

— certificate for paid taxes, contributions and other public expenses from a competent body from the country where the economic operator is registered;

— certificate that no sanction for prohibition for participating in public procurement procedures, awarding public procurement contracts and contract for public-private partnerships is issued by the Sanction register for committed criminal acts of legal entities;

— certificate that no sanction for permanent or temporary prohibition for performing of separate activity is issued by the Sanction register for committed criminal acts of legal entities;

— Certificate that no offence sanction has been passed by verdict in force — prohibition from practising a profession, activity or duty that is temporary prohibition from exercising a certain duty.

— Evidence for Universities/Faculties of Medicine, as well as evidence to proof ranging as top 100 on the Shanghai 2013 and 2014 List Academic Ranking of World Universities list (http://www.shanghairanking.com/ or alternative established organizations by the universities/faculties.

— Statement that experienced auditors who will perform clinic audit services on 7 University Clinics in R. M. will be provided, as follows:

1. University Clinic for traumatology, orthopedics, anesthesiology, reanimation, intensive care and urgent centre (6 experts);

2. University Clinic for gynecology and obstetrics (3 experts);

3. University Clinic for abdominal surgery (3 experts);

4. University Clinic for cardiology (3 experts);

5. University Clinic for toracal-vascular surgery (3 experts);

6. University Clinic for children diseases (6 experts);

7. University Clinic for infection diseases (3 experts).

— Medical Universities to state that regular professor services, at least 10 years experienced in the relevant field and minimum 10 publications in science magazines within the relevant field will be offered;

— Statement for provided team for each of the 7 relevant fields, including experienced experts in 3 various sub-specialties (the team leader, additionally must have management experience or equivalent experience in management and evaluation of relevant health organizational charts, experience in the relevant audit field, institutional capacity building and policy development in the relevant area, or experience in same or similar projects)

— To state that the national standards (clinic paths) as well as the standards of NICE (National Institute for Clinical Care and Excellence) will be applied in the clinic audit results.

Минимално ниво(а) на стандарди:

—Universities/Faculties of Medicine, which are ranged as top 100 on the Shanghai 2013 and 2014 List Academic Ranking of World Universities list (http://www.shanghairanking.com/ or alternative established organizations by the universities/faculties.

— To offer experienced auditors who will perform clinic audit services on 7 University Clinics in R. M.:

1. University Clinic for traumatology, orthopedics, anesthesiology, reanimation, intensive care and urgent centre (6 experts);

2. University Clinic for gynecology and obstetrics (3 experts);

3. University Clinic for abdominal surgery (3 experts);

4. University Clinic for cardiology (3 experts);

5. University Clinic for toracal-vascular surgery (3 Experst);

6. University Clinic for children diseases (6 experts);

7. University Clinic for infection diseases (3 experts).

— The Medical Universities must offer regular professor services, at least 10 years experienced in the relevant field and minimum 10 publications in science magazines within the relevant field.

Правна форма што треба да ја преземе групата економски оператори на која ќе се додели договорот:

Every interested local and international legal entity and individual bidder, which is registered for performing activity related to the procurement subject and that has obtained the tender documentation from the Contracting Authority is entailed to submit a bid.

Group of bidders which are not obliged to merge in suitable legal form is also entitled to submit a bid. The Contractual party shall not request the group of bidders to merge in suitable legal form, unless the very same is elected as procurement leader.

The group tender must be enclosed with joint venture agreement under which the members are both mutually and before the contracting authority obliged to perform the procurement contract. This contract must mandatory contain the following data: the team leader who shall submit the tender and represent the group, the group member who shall sign the public procurement contract, the member who shall provide the bank guarantee on behalf of the group members, the member who shall issue the payment invoice and bill, short description of the group members liabilities for contract enforcement.
