— Statement by the economic operator than it has not been sentenced, in the last 5 years, with effective court decision for participation in criminal organization, corruption, fraud or money laundering;

— Certificate that it is not subject to bankruptcy procedure issued by competent authority;

— Certificate that it is not subject to liquidation procedure issued by competent authority;

— Certificate for paid taxes, contributions and other public duties by the competent authority in the country where that economic operator is established;

— Certificate from the Legal Entities Penal Registry that it is not being imposed a side sanction —…

… prohibition to participate in public competitions, public procurement contract award and public-private partnership contract award procedures;

… temporary or permanent prohibition to perform certain professional activity;

— Certificate that it is not being sanctioned for misdemeanor with an effective court decision, resulting in prohibition for pursuing professional activity or duty, i.e. temporary prohibition for performing professional activity;

To prove its ability for performing professional activity the economic operator submits:

— Document for registered activity;

Document for registered activity — DRD Form, issued by the Central Registry of R. Macedonia, or appropriate document issued by competent authority in the country where economic operator is registered;

— Valid authorization for wholesale of medical devices issued by the Ministry of Health of Republic of Macedonia or appropriate document issued by competent authority in the country where the economic operator is registered.

2.3.2 Proof issued.

Техничка и професионална способност:

— The object of procurement — the reagents should be approved for sale on the territory of Republic of Macedonia in accordance with the Law on drugs and medical devices i.e., to be registered;

— Have evidence which confirms the requirements of the technical specifications i.e., the required characteristics of the goods that are object of procurement,

— The reagents and the supplies must match the device for which the offer is submitted i.e., the reagents and supplies to be equivalent to the reagents for use of the appropriate instrument for which the offer is submitted (appropriate to the PART).

Минимално ниво(а) на стандарди:

— Decision (approval) that the reagents are approved for sale on the territory of Republic of Macedonia in accordance with the Law on drugs and medical devices;

— Catalogues or other documents that will prove the requirements of the technical specifications of the main features of the products that are object of procurement with clearly marked pages of the reagents and supplies;

— Statement by the economic operator that the reagents and supplies match or are equivalent to the reagents for the requested instrument (appropriate to the PART).
