Statement by the economic operator that it has not been announced, in the last 5 years, effective court decision for participation in criminal organization, corruption, fraud or money laundering:

— Certificate that it is not subject to bankruptcy procedure issued by competent authority;

— Certificate that it is not subject to liquidation procedure issued by competent authority;

— Certificate for paid taxes, contributions and other public duties by the competent authority in the country where that economic operator is established;

— Certificate from the Register of penalties for committed criminal acts of legal entities that a minor penalty…

Bidder is required to possess License A for performing supervision of works on first (I) category structures, issued by the competent authority in the country as per the Construction Law in force (Official Gazette of Republic of Macedonia no. 130/09, 124/10, 18/11, 36/11, 54/11, 13/12, 144/12, 25/13, 79/13, 137/13, 163/13, 27/14, 28/14, 42/14, 115/14, 149/14 and 187/14). (applies to each member of the group in case bid is submitted by a group of economic operators – JV).

Економска и финансиска состојба:

— Total income of 10 000 000 MKD in the last 3 years (aggregate amount for 2012, 2013, and 2014).

The economic operator shall prove that he meets the requirements for Economic and financial status by furnishing the following:

— Copy of balance sheet report and data on income statements for 2012, 2013 and 2014, certified by a competent authority, i.e. audited balance sheets or abstracts of the report on balance sheets, where balance sheet publication is required under the law of the country where the economic operator is registered;

— For Foreign Tenderers: Copies of auditors' reports, which shall be positive and shall be issued by a reputable foreign institution registered to provide audit services. Auditor reports should contain data about the overall economic operation of the Foreign Tenderer (for the last 3 years).

Техничка и професионална способност:

— One completed contract for performing supervision of works on construction, reconstruction or rehabilitation of state roads, main streets and boulevards in settlements, or airports, or interchanges, according to FIDIC contracting terms for construction works in the last 3 (three) years in an amount of minimum 1 000 000 MKD (denars).

— Notification on contract items that the economic operator intends to subcontract to third parties.

Key Personnel:

Engineer's Representative

— License A for a supervising engineer in civil engineering as per the Construction Law in force (Official Gazette of Republic of Macedonia no. 130/09, 124/10, 18/11, 36/11, 54/11, 13/12, 144/12, 25/13, 79/13, 137/13, 163/13, 27/14, 28/14, 42/14, 115/14, 149/14 and 187/14);

— University Diploma in Civil Engineering (BSc CEng);

— Work experience of at least 7 (seven) years in civil engineering;

— Experience in supervision on at least 1 (one) completed project as an Engineer's Representative, or a Key Staff member, or a Project Manager of construction, or reconstruction, or rehabilitation of state roads, main streets and boulevards in settlements, or airports, or interchanges, implemented under FIDIC terms and conditions of the Contract on Works, in any country.

Assistant Engineer's Representative

— Work experience of at least 5 (five) years in civil engineering;

— Experience in supervision on at least 1 (one) completed project as a Supervising Engineer, or a Project Manager of construction, or reconstruction, or rehabilitation of state roads, main streets and boulevards in settlements, or airports, or interchanges, implemented under FIDIC terms and conditions of the Contract on Works, in any country.

Expert in Environment Protection - Diploma in the field of environmental engineering, civil engineering, architecture, technical — technological sciences, ecology, or other appropriate diploma in natural sciences related to the environment protection;

— Certificate of an Expert in Environmental Impact Assessment issued by a competent authority as per the laws in the Republic of Macedonia;

— 3 (three) years of work experience in environment protection.

Surveyor

— Minimum high (secondary) school degree for a geodetic technician — surveyor;

— Minimum 5 (five) years of work experience in construction, reconstruction or rehabilitation on projects in civil engineering.

The economic operator shall prove that he meets the requirements for Technical and professional competence by furnishing the following:

— Written confirmation of a high-quality completion of a contracting agreement verified by the Contracting Authority, and a copy of the Contract.

— Copies of M1 and M2 employment forms, or other corresponding forms issued by a competent institution in the domicile country of the economic operator, as well as copies of diplomas;

For Engineer's Representative:

— Copy of License A for supervising engineer in civil engineering as per the Construction Law in force (Official Gazette of Republic of Macedonia no. 130/09, 124/10, 18/11, 36/11, 54/11, 13/12, 144/12, 25/13, 79/13, 137/13, 163/13, 27/14, 28/14, 42/14, 115/14, 149/14 and 187/14);

— Copy of the Diploma, and

— Filled out biography form (provided in bidding documents) of proposed experts and copy of M1/M2 employment form, or other corresponding evidence of employment issued by a competent authority in the economic operator's domicile country.

For Assistant Engineer's Representative:

For Environment Protection Expert

— Copy of the Certificate of an Expert in Environmental Impact Assessment issued by a competent authority as per the laws in the Republic of Macedonia;

For Surveyor

— Copy of the Diploma or other evidence of completed education;
