Statement by the economic operator than it has not been sentenced, in the last 5 years, with effective court decision for participation in criminal organization, corruption, fraud or money laundering;

Certificate that it is not subject to bankruptcy procedure issued by competent authority;

Certificate that it is not subject to liquidation procedure issued by competent authority;

Certificate for paid taxes, contributions and other public duties by the competent authority in the country where that economic operator is established;

Certificate from the Legal Entities Penal Registry that it is not being imposed a side sanction —…

… prohibition to participate in public competitions, public procurement contract award and public-private partnership contract award procedures;

… temporary or permanent prohibition to perform certain professional activity;

Certificate that it is not being sanctioned for misdemeanor with an effective court decision, resulting in prohibition for pursuing professional activity or duty, i.e. temporary prohibition for performing professional activity;

To prove its ability for performing professional activity the economic operator must submit:

— Document for registered activity;

Document for registered activity as evidence that it is registered as a physical or legal entity for performing the activity related to the subject of the contract or proof that it belongs to a relevant professional association in accordance with the regulations of the country where it is registered.

To be able to wholesale medical devices the legal entity must submit — Permission for wholesale of medical devices issued by the Ministry of Health (in accordance with the Law of public procurement of drugs and medical materials).

Минимално ниво(а) на стандарди:

Balance sheet report certified by a competent authority, i.e., audited balance sheet, or extracts from the balance report in case when the publishing of the balance report prescribed by law, in the country where the economic operator is registered.

Statement of the total turnover of the enterprise (data from the income statement issued by a competent authority, i.e., audited income statement) and, where appropriate, of turnover in the area covered by the contract of public procurement and for the last 3 financial years the most and for which such data are available, depending on the date on which the enterprise was set up or started work.

Balance sheet and report of the income statement certified by a competent authority, i.e., audited balance sheets or extracts from the balance sheets where publication of the balance sheet is required under the law of the country in which the economic operator is registered.

Successful delivery of goods identical or closely related to the object of public procurement;

For the offered goods the economic operator to have Decision for wholesale on the territory of Republic of Macedonia;

To have samples of the offered products;

To have catalogs and pictures of the same;

Economic operator to be prepared to deliver sutures identical to the offered samples;

To offer products with Certificates issued by independent institutions which confirm that the economic operator complies with particular standards of quality based on relevant European and international standards; or equivalent Certificates issued by Institutions based in EU or equivalent;

List of main deliveries from the bidders or of the product candidates that are subject of the delivery, with values, dates, buyers (contracting authorities or economic operators), providing a minimum of 1 confirmation for completed service delivery issued by recipients, or if such a confirmation can not be provided for reasons beyond the control of the economic operator, then its statement for completed deliveries needs to be submitted;

Decision issued by the Ministry of Health of RM for each of the products subject to the procurement that are registered on the territory of RM and have an authorization for wholesale of medical materials on the territory of RM;

It is mandatory to submit 3 pieces from each sample, because of testing of the same in order to confirm that they possess the minimum technical characteristics as in the technical specification (with no obligation to return or payment), description and photos of products that are subject to delivery and whose reliability economic operator is obliged to verify if requested by the contracting authority). To submit a statement, that for the duration of the contract, will deliver products identical to the submitted samples.

The samples need to fully match the given technical description and fully correspond to the functionality that they need to accomplish. In the archive of the contracting authority, the economic operator shall submit all the samples required in the technical specifications in a closed package which states:

— The correct address of the contracting authority;

— Information regarding the number of the advertisement of the public procurement contract and the date it was published;

— in the upper left corner write warning ‘Do Not Open — samples’ not to be opened before the time and date of bid opening;

— In the package a list of contents of the package must be submitted.

If the package is not closed and marked as required, the contracting authority does not take responsibility for the misplacement or premature opening of the same. The packages that are open early for failure of following the conditions for closing and marking the same it will be rejected;

To submit catalogues or photos

To submit a statement of the bidders that for the duration of the contract, will deliver products identical to the submitted samples

Copies of Certificates issued by the competent authorities for control of the quality with recognized competence, that test the suitability of the offered products clearly identified by references to specifications or standards as follows:

— EC / CE certificate — which proves that the product matches the prescribed EU standards or equivalent;

— Declaration of conformity or equivalent;

— Free sale certificate or equivalent.
