IV.2.1) In order to prove its personal status, the economic operator should submit the following documents:

— Statement of the economic operator that no effective verdict has been issued against the economic operator within the past 5 years for participation in a crime organization, corruption, fraud or money laundering;

— Confirmation stating that no bankruptcy procedure has been initiated by a competent body;

— Confirmation stating that no liquidation procedure has been initiated by a competent body;

— Certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered;

— Confirmation from the Register of Penalties for Committed Criminal Offences of Legal Entities proving that no secondary penalty —…

… ban on participating in open call procedures, awarding public procurement contracts and contracts on public-private partnership has been pronounced;

… temporary or permanent ban on performing particular activity has been pronounced;

— Confirmation stating that no misdemeanour sanction — ban on the performance of the profession, activity or duty, i.e. temporary ban on the performance of a particular activity has been pronounced with an effective verdict.

IV.2.2) Capability of the economic operator for performing professional activity:

— Document for registered activity:

In order to prove the capability for performing professional activity, the economic operator should submit a document for registered activity, as a proof that the economic operator is registered as a natural person or legal entity for performing the activity related to the subject of the public procurement contract or proof of membership in the relevant professional association in accordance with regulations in the country where the economic operator is registered.

IV.2.3) Economic and financial status of the economic operator:

Necessary documents for proving the economic status of the economic operator:

— Balance sheet verified by a competent body i.e. an audited balance sheet, or extracts from the balance sheet, in cases when the disclosure of the balance sheet is regulated by law in the country where the economic operator is registered.

Minimum criteria for determining the economic and financial status of the economic operator:

— Positive financial operation in the last 3 years (2013, 2014 and 2015).

Техничка и професионална способност:

IV.2.4) Technical or professional capability of the economic operator:

Necessary documents for proving the technical or professional capability of the economic operator:

— Copies of realized contracts and certificates issued by the investors for qualitative and timely performed works upon such contracts;

— Statement of the economic operator that it will appoint an architect or civil engineer as the chief construction engineer;

— Head of construction, architect or civil engineer. The following must be enclosed to the Statement: CV, A-Authorization for construction engineer issued by the Chamber of Authorized Engineers of the Republic of Macedonia, Form M1/M2 or certificate from competent body for the person employed at the economic operator or statement of the person not employed at the economic operator that the person consents to be hired on such position;

— Statement confirming that the economic operator will hire at least 5 (five) graduated engineers (architect, civil engineer, hydro engineer, electrical engineer and mechanical engineer) with mandatorily enclosed documents: CV, A-Authorization for construction engineer issued by the Chamber of Authorized Engineers of the Republic of Macedonia, Form M1/M2 or certificate from competent body for the person employed at the economic operator or statement of the person not employed at the economic operator that the person consents to be hired on such position;

— List/Certificate issued by the Employment Agency of the Republic of Macedonia for persons qualified to perform the activity related to the subject of the public procurement contract at the time of bid submission, not older than 15 days calculated as of the deadline for bid submission backwards;

— Statement with which the economic operator guarantees that it possesses the necessary equipment and machines, i.e. that it will provide such equipment and put it at disposal in the course of contract realization;

— Notification for the contract elements the economic operator intends to assign to the subcontractor.

Минимално ниво(а) на стандарди:

Minimum criteria for determining the technical and professional status of the economic operator:

— The economic operator should have constructed at least 2 facilities (realized contracts) for building construction, with total (summed) square surface no less than 10 000 m

— The economic operator should possess qualified staff (persons employed at the economic operator or to hire persons from other economic operators) for realization of the subject of procurement as follows:

1. To appoint chief construction engineer — head of construction architect or civil engineer, with previous experience as chief construction engineer of at least 2 (two) constructed facilities/buildings with A-Authorization for construction engineer, issued by the Chamber of Authorized Engineers of the Republic of Macedonia;

2. At least 5 (five) graduated engineers as follows: architect, civil engineer, hydro engineer, electrical engineer and mechanical engineer with A-Authorization for construction engineer, issued by the Chamber of Authorized Engineers of the Republic of Macedonia;

3. At least 20 (twenty) employees at the time of bid submission, qualified for performing the activity related to the subject of the public procurement contract (including the qualified staff — engineers).

— The Economic operator should possess equipment and machines necessary for realization of the contract subject.
