IV.2.1) In order to prove its personal status, the economic operator should submit the following documents:

— Statement of the economic operator that no effective verdict has been issued against the economic operator within the past 5 years for participation in a crime organization, corruption, fraud or money laundering;

— Confirmation stating that no bankruptcy procedure has been initiated by a competent body;

— Confirmation stating that no liquidation procedure has been initiated by a competent body;

— Certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered;

— Confirmation from the Register of Penalties for Committed Criminal Offences of Legal Entities proving that no secondary penalty —…

… ban on participating in open call procedures, awarding public procurement contracts and contracts on public-private partnership has been pronounced;

… temporary or permanent ban on performing particular activity has been pronounced;

— Confirmation stating that no misdemeanour sanction — ban on the performance of the profession, activity or duty, i.e. temporary ban on the performance of a particular activity has been pronounced with an effective verdict.

IV.2.2) Capability of the economic operator for performing professional activity:

— Document for registered activity.

In order to prove the capability for performing professional activity, the economic operator should submit a document for registered activity, as a proof that the economic operator is registered as a natural person or legal entity for performing the activity related to the subject of the public procurement contract or proof of membership in the relevant professional association in accordance with regulations in the country where the economic operator is registered.

— Proof issued by a competent body for meeting the special conditions for performing the activity regulated pursuant to the law which refer to the contract subject.

— ‘A’ license for building 1

Економска и финансиска состојба:

Necessary documents for proving the economic status of the economic operator:

— Balance sheet verified by a competent body, i.e. an audited balance sheet, or extracts from the balance sheet, in cases when the disclosure of the balance sheet is regulated by law in the country where the economic operator is registered.

Минимално ниво(а) на стандарди:

— Positive financial operation in the last 3 (three) years (2014, 2013 and 2012).

Техничка и професионална способност:

— Copies of realized contracts and certificates issued by the investors for high-quality and timely performance of the works upon said contracts;

— Statement of the economic operator that it will…

… appoint an architect or civil engineer as chief construction engineer — head of the construction, architect or civil engineer. The following must be enclosed to the Statement: CV (Curriculum Vitae), ‘A’ authorization for construction engineer issued by the Chamber of Certified Architects and Certified Engineers of the Republic of Macedonia, M1/M2 Form or certificate from competent body regarding the person employed at the economic operator or statement of the person not employed at the economic operator stating that he/she consents to be hired for this position;

… hire minimum 5 (five) graduated engineers with the following enclosed documents: CV ((Curriculum Vitae), ‘A’ authorization for construction engineer issued by the Chamber of Certified Architects and Certified Engineers of the Republic of Macedonia, M1/M2 Form or certificate from competent body regarding the person employed at the economic operator or statement of the person not employed at the economic operator stating that he/she consents to be hired for this position;

— List/Certificate issued by the Employment Agency of the Republic of Macedonia, for employed persons qualified to perform the activity related to the subject of the public procurement contract at the time of submission of the tender bid;

— Statement of the economic operator which guarantees that the economic operator possesses the necessary equipment and machines, i.e. that it shall provide and use thereof for engagement purposes during the realization of the contract;

— Notification for the contract elements the economic operator tends to assign to the subcontractor.

Минимално ниво(а) на стандарди:

— The economic operator should have constructed minimum 2 (two) facilities (realized contracts) i.e. 1

— The economic operator should possess highly qualified personnel (employed persons at the economic operator or persons hired from another economic operator) hired for the realization of the subject of procurement, as follows:

i. Chief construction engineer — head of construction, architect or civil engineer, with previous experience as chief construction engineer of minimum 2 (two) performed facilities/buildings, whose surface is cumulatively not less than 5 000 m

ii. Minimum 5 (five) graduated engineers (architects, civil engineer, hydro engineer, electrical engineer and mechanical engineer), with ‘A’ authorization issued by the Chamber of Certified Architects and Certified Engineers of the Republic of Macedonia;

iii. Minimum 20 (twenty) employed persons at the time of submission of the tender bid, qualified to perform the activity related to the subject of the public procurement contract (including the experts — engineers);

— The economic operator should have at disposal the equipment and machines necessary for performance of the planned works, i.e. provide thereof.
