Procurement of service for servicing, maintenance and repair of ambulance motor vehicles for emergency care with installed medical equipment for the needs of the vehicle fleet of PHI Health House Skopje –Skopje. Vehicles subject to servicing are of Renault type and they are equipped with the following medical equipment — portable aspirator, defibrillator, gluco-meter, wall device for measurement of blood pressure and portable system for oxygen support.

— Statement of the economic operator that in the past five years it was not rendered a final judgment for participation in criminal organization, corruption, fraud or money laundering;

— Confirmation by a competent organ that a…

… bankruptcy procedure was not opened;

… liquidation procedure was not opened;

— Confirmation for paid taxes, contributions and other public duties by a competent organ in the country where the economic operator was registered;

— Certificate from the Registry of penalties for crimes committed by legal persons that minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him;

— certificate from the Registry of penalties for crimes committed by legal persons that a minor penalty for temporary or permanent prohibition of a specific activity has not been imposed over him;

— confirmation that a final judgment — prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.

Capacity for performing professional activity of economic operator:

— Document for registered activity.

In order to prove its capacity for performing professional activity, the economic operator should submit a document for registered activity as an evidence that it is registered as a legal entity for performing the activity related to the public procurement subject or an evidence that it belongs to relevant professional association in accordance to the regulations of the country where it is registered.

Техничка и професионална способност:

List of main deliveries (concluded contracts and confirmed contracts) in the last three years, with values, dates, contracting parties, by providing minimum one confirmation for realized contract for provided service of servicing maintenance and repair of ambulance motor vehicles for emergency medical care with installed medical equipment) issued by the beneficiary of the service. If the economic operator performs the activity for less than 3 years, it should submit the documents for the years it has been registered. If such confirmation may not be provided due to reasons out of control of the economic operator, it should submit a statement for realized deliveries.

Experience in relevant previous rendered services that were successfully realized.
