For personal condition confirmation, economic operator should submit these documents:

— Statement of economic operator that in the last 5 years there was no effective conviction for participation in criminal organization, corruption, fraud or money laundering;

— Certificate that any competent authority has initiated no open…

… bankruptcy procedure;

… liquidation procedure;

— Certificate for paid taxes, contributions and other public payments by competent authority in the country where the economic operator has been registered in;

— Certificate from the Register of Offences for Crimes Committed by Legal Persons that there is no minor penalty that prohibits participation in procedures for public announcement, awarding contracts on public procurements and contract for public-private partnership;

— Certificate from Register of Penalties for Crimes Committed by Legal Persons that there is no effective conviction for temporary or permanent prohibition to perform certain activity;

— Certificate that there is no effective conviction by which infringement sanction has been imposed on — interdiction for professional activities, activities or duties, i.e. temporary interdiction of separate activity realization.

Capability for professional activity performance of the economic operator:

— Document for registered activity DRD form;

— In order to prove the ability to perform professional activity, the economic operator should submit a document for registered activity as proof that it is registered as natural or legal entity to perform the activity related to the subject of this public procurement or proof that it belongs to appropriate professional association in accordance with the regulations of the country he is registered in.

Required documents which support the economic standing of the economic operator:

— Appropriate bank statements, such as the 1

— Balance sheet certified by competent authority i.e. revised balance sheet or excerpts of the balance sheet in cases when the publication of balance sheet is prescribed by law in the country where the economic operator is registered in;

— Statement of the overall turnover of the enterprise (data from the profit and loss account issued by competent authority i.e. revised profit and loss statement) with minimum annual turnover not less than 3 000 000 MKD and where appropriate, from the turnover in the area covered by the public procurement contract, mostly for the last 3 financial years for which such information are available, depending on the date on which the enterprise has been established or stared operating and depending on such information's availability.

Техничка и професионална способност:

Required documents for proving the technical or the professional capability of the economic operator:

Reference list of main shipments of the bidders performed in the last 3 years and of those in progress, with subject, date, purchaser (contracting authorities or economic operators), with description of used hardware and specifications for the software used to mark and process the research data, by providing confirmation for performed deliveries issued by the receivers or, if unable to provide such confirmations for reasons out of the economic operator' control, only by his statement for performed deliveries and agreements in progress.

— Statement on technical equipment and ability of the economic operator and for other potentials he disposes with in order to perform the specific services and provide quality which shall mandatory contain:

a) Information on the hardware which will be used for measuring, by type, quantity and characteristics;

b) Specification (operating description and characteristics) on the software which will be used for measuring;

c) Information on measures that will be used for internal and external (current inspection by the contracting authority) quality control, for the duration of the agreement.

The selected most favourable bidder shall be obliged to provide performance guarantee in the amount of 10 % of the contract value, including VAT.
