— Confirmation that there are no bankruptcy proceedings opened against the operator by a competent authority.

— Confirmation that there are no liquidation proceedings opened against the operator by a competent authority.

— Receipt for paid taxes, contributions and other public duties by the competent authority in the country where the operator is registered.

— Certificate from the Registry of penalties for criminal acts by legal entities that minor penalty…

Економска и финансиска состојба:

Appropriate liability insurance for damage by an insurance company in the Republic of Macedonia, which by their work can create a minimal total income for the last 3 years for the investors or third parties (2014, 2015 and 2016) in amount of at least 300 000 000 MKD.

Минимално ниво(а) на стандарди:

— insurance liability policy for damage by an insurance company in the Republic of Macedonia on the name of the company which owns the license for the current calendar year which cannot be lower than 10 000 EUR in MKD equivalent according to Article 41 from the Law on civil construction, Official Gazette of The Republic of Macedonia.

— Balance sheet report certified by the competent authority i.e. audited balance sheet or excerpts from the balance sheet report in such cases when publishing balance sheet is regulated by law within the country where the operator has been registered, for the last 3 years (2014, 2015 and 2016).

— statement of the total turnover of the company (Income Statement information issued by the competent authority i.e. audited Income Statement) for the last 3 years (2014, 2015 and 2016) and where appropriate, from the income in the area covered with the public procurement agreement mostly for the last three financial years for which such information is available depending on the date on which the company had been founded or started its work and on the availability of such information.

Техничка и професионална способност:

— relevant experience from previous performed services from same type which had been successfully realized in the last 3 years and at least 6 realized contracts in the area covered by the public procurement contract out of which at least 3 contracts with public health institutions, in total value of minimum 60 000 000 MKD excluding VAT.

— minimum number of expert persons responsible for subject procurement realization:

— minimum 3 graduated engineers from which: 1 civil engineer with Authorization A for construction with at least 10 (ten) years professional experience, 1 architect engineer with Authorization A with at least 10 (ten) years professional experience, and 1 engineer architect with Authorization B with minimum of 5 (five) years professional experience with mandatory attached evidence of Safety and Health at Work Training according Article 31 from the Law on Safety and Health at Work, Official Gazette No.92/07,

— minimum 11 individuals working as technical staff who will be involved in the implementation of contract subject (technicians and construction workers) with mandatory attached evidence of Safety and Health at Work Training according Article 31 from the Law on Safety and Health at Work, Official Gazette No.92/07,

— appropriate technical equipment and capacity of the economic operator for performing the specified services and providing quality assurance such as:

— minimum 2 universal construction machines (skip) out of which one will be continuously within Contracting Authority area,

— minimum 3 trucks out of which 1 truck — kipper with minimum of 5 tons load,

— minimum 1, 3-ton roller,

— minimum 1 asphalt cutter machine,

— minimum 1 concrete cutter machine.

Минимално ниво(а) на стандарди:

Main service list of the bidders or candidates performed in the last three years by obtaining minimum of 6 contracts for service performance which are subject of procurement out of which minimum 3 contracts with Public health institutions, in total value of minimum 60 000 000 MKD excluding VAT by providing confirmations for successfully provided services of same type issued by other Contracting Authorities where such services were provided previously, or if it is not possible to obtain such confirmations by reasons beyond the economic operator control, only its statement for performed services.

— Statement from the economic operator for the professional staff who will be engaged for realization of the subject of procurement, listed by name and surname and the following to be obtained: — M1/M2 Form for the professional staff who will be engaged for realization of the subject of procurement, Higher education diploma for the engineers, Authorization A or B for performance, — Safety and Health at Work Certificate, — Statement for engaged technical staff and technical bodies regardless if they belong to the economic operator, listed by name and surname and provided Safety and Health at Work Certificate for the same. — Statement for technical and professional capacity of the economic operator for performing specific services and providing quality assurance that the economic operator is in possession of: — minimum 2 universal construction machines (skip) from which one will be continuously ranked in the contracting authority area, — minimum 3 trucks from which 1 truck — kipper with minimum 5 tons load, — minimum one 3-ton roller, — minimum 1 asphalt cutter machine, — minimum 1 concrete cutter machine.
