Statement of the economic operator proving that in the last 5 years has had an effective sentence for participation in a criminal organization, corruption, fraud or money laundering; — Certificate that a bankruptcy procedure has not been initiated by the competent body; — Certificate that a liquidation procedure has not been initiated by the competent body; — Certificate of paid taxes, benefits and other public duties, issued by the competent body in the country where the economic operator is registered; — Certificate from the Registry of penalties for committed acts of crime by legal entities that the economic operator has not been issued an additional penalty of a ban on participating in procedures for public call for bids, for awarding public procurement contracts and for contracts on public — private partnership; — Certificate from the Registry of penalties for committed acts of crime by legal entities that the economic operator has not been issued an additional penalty of a temporary or permanent ban on performing a certain activity.

2.3.1 To prove the capability of doing professional
activity, the economic operator should submit a document for registered activity as proof that it is registered as a legal entity for the activity related to the subject of the contract or evidence that it belongs to professional association in accordance with the regulations of the country where it is registered.

Certificate issued by the competent authority for the fulfillment of specific conditions for the activity prescribed under law relating to the subject matter of the contract.
— Decision that is registered in the register of legal entities for wholesale of medical devices issued by the Agency for medicines and medical supplies under the Law on Medicines and Medical Devices Article 126, paragraph 2.

The economic operator shall prove the fulfillment of the minimum requirements in sub-section 2.5.1 by submitting a statement for the qualified technical and educated personnel, accompanied by a certificate for the person in charge of installing the equipment. — Statement for secured technical and educated personnel, accompanied by a certificate for the person in charge of maintaining the equipment. — a statement for qualified technical and educated personnel, accompanied by a certificate for the person in charge of training the persons who will work with the equipment. For registered medical devices — Decision for placing a medical device on the market or temporary or permanent approval for parallel import, in accordance with the Law on Medicines and Medical Devices. For unregistered medical devices — Import Permit (Intervention Import Authorization) in accordance with the Law on Medicines and Medical Devices.

2.5.1 In order to qualify as capable of carrying out the relevant public procurement contract in terms of its technical or professional capability, the economic operator must meet the following minimum requirements — qualified technical and educated personnel, with a certificate for at least one trained person In charge of installing the equipment. — expert technical and educated personnel, with a certificate for at least one trained person in charge of training of the persons who will work with the equipment — expert technical and educated personnel, with a certificate for at least one trained person in charge of maintenance of the equipment. For registered medical devices — The economic operator shall provide for the offered goods an authorization for placing a medical device on the market or temporary or permanent approval for parallel import, in accordance with the Law on medicines and medical devices. For unregistered medical devices — The economic operator shall provide for the offered goods a document permitting import (interim import clearance) in accordance with the Law on Medicines and Medical Devices.
