In order to prove its personal condition the economic operator is obliged to submit the following documents:

— Statement of the economic operator that in the past five years it was not rendered a final judgment for participation in criminal organization, corruption, fraud or money laundering.

— Confirmation by a competent organ that a…

… bankruptcy procedure was not opened.

… liquidation procedure was not opened.

— Confirmation for paid taxes, contributions and other public duties by a competent organ in the country where the economic operator was registered.

— Certificate from the Registry of penalties for crimes committed by legal persons that…

… minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him.

… a minor penalty for temporary or permanent prohibition of a specific activity has not been imposed over him.

— Confirmation that a final judgment — prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.

Capacity for performing professional activity of economic operator:

Document for registered activity.

In order to prove its capacity for performing professional activity, the economic operator should be registered as natural or legal entity for performing the activity related to the public procurement subject or to be a member of a relevant professional association in accordance to the regulations of the country where it is registered. The economic operator shall prove the meeting of the minimum conditions by submitting a document for registered activity issued by the Central Register of the Republic of Macedonia, as an evidence that it has been registered as natural or legal entity for performing the activity related to the public procurement subject or an evidence that it is a member of a relevant professional association in accordance to the regulations of the country where it is registered. Evidence issued by a competent organ for meeting the special conditions for performing the activity prescribed pursuant to law that are related to the subject of the contract. The economic operator should be registered for performing activity of wholesale of medical devices. The economic operator shall prove the meeting of minimum conditions by submitting a valid Decision for wholesale of medical devices on the territory of the Republic of Macedonia issued by the Agency for Medicines and Medical Devices of the Republic of Macedonia.

Техничка и професионална способност:

1. List of main deliveries related to the same or similar equipment to the one defined in the technical specification, realized in the past three years, by providing confirmations for the deliveries made issued by the receivers, or if such confirmations may not be provided due to reasons out of control of the economic operator, only its statement for deliveries made. 2. Statement and enclosure of certificate for provided professional technical and educated personnel responsible for installing the equipment, 3. Statement and enclosure of certificate for provided professional technical and educated personnel responsible for equipment maintenance with technical assistance, 4. Statement and enclosure of certificate for provided professional technical and educated personnel responsible for training of the personnel that will work with the equipment, 5. Statement of the economic operator that it is equipped technically and with personnel for regular service of the equipment in the warranty period. 6.For registered medical devices -Decision for putting into market circulation of medical device or temporary or permanent approval for parallel import, pursuant to the Law on Medicines and Medical Devices. 7. For unregistered medical devices — Import license (approval for intervention import) pursuant to the Law on Medicines and Medical Devices.

Минимално ниво(а) на стандарди:

1. Realized contracts for medical equipment procurement.

2. Professional technical and educated personnel responsible for installing the equipment.

3. Professional technical and educated personnel responsible for equipment maintenance with technical assistance.

4. Professional technical and educated personnel responsible for training of the personnel that will work with the equipment.

5. Technical equipment and capacity of the economic operator for regular service of the equipment in the warranty period.

6. For registered medical devices -the economic operator, for the offered goods, should have provided approval for putting into circulation on the market of medical device or temporary or permanent approval for parallel import, pursuant to the Law on Medicines and Medical devices.

7. For unregistered medical devices — the economic operator, for the offered goods, should have provided a document allowing import (approval for intervention import) pursuant to the Law on Medicines and Medical devices.
