In order to prove its personal status, the economical operator should submit the following documents:

— statement of the economic operator stating that no effective verdict has been pronounced in the past five years for participation in criminal organization, corruption, fraud or money laundering;

— confirmation that no bankruptcy procedure has been initiated by a competent authority;

— confirmation that no liquified ation procedure has been initiated by a competent authority;

— confirmation for paid taxes, contributions and other public fees from the authority competent in the country where the economic operator has been registered;

— confirmation from the Registry for penalties for committed criminal acts of the legal entities stating that no secondary penalty,…

… ban on participation in open call procedures, on awarding of contracts for public procurement and contracts on public-private partnership has been pronounced;

… temporary penalty or permanent ban on the performance of particular activity has been pronounced;

— confirmation stating that no misdemeanour sanction — ban on the performance of the profession, activity or obligation, i.e. temporary ban on the performance of a particular activity has been pronounced with the effective verdict.

Capability for performing professional activity of the economical operator:

— Document for registered professional activity.

Економска и финансиска состојба:

The following documents are necessary in order to prove the economic capability of the economic operator:

— statement of the overall turnover of the company (information on the profit and loss statement issued by the competent authority, i.e. audited profit and loss statement) and where appropriate, of turnover in the area covered by the public procurement contract, mostly for the last 3 financial years for which such information is available, depending on the date on which the company was founded or started work and depending on the availability of such information.

Documents necessary to prove the technical and professional capability of the economic operator:

Part 1 — Procurement of fuels — Subsidiary "Tikvesh" — Kavadarci

— A list of successfully completed contracts related to the subject of procurement in the last 3 years, including values of contracts, dates of concluding, buyers (contracting authorities or economic operators), by providing a certificates of performed deliveries issued by recipients, or, if such certificates cannot be provided for reasons beyond the control of the economic operator, only with his statement on performed deliveries

— a statement on the use of cashless payment system,

— a statement of disposing with, i.e. providing at least one gas station within a radius of maximum 10 km from the headquarters of Subsidiary “Tikvesh” — Kavadarci on Str. Industriska bb — Kavadarci.

The proof are prepared by the economic operator himself and they should be digitally signed with a digital certificate by the responsible or authorized person of the economic operator.

Part 2 — Procurement of fuels — Subsidiary "Juzen Vardar" –Gevgelija

— a statement of disposing with, i.e. providing at least one gas station within a radius of maximum 10 km from the headquarters of the Subsidiary on Str. Sedmi Noemvri no.14- Gevgelija.

Part 3 — Procurement of fuels — Subsidiary "Crni Drim" — Ohrid

— a statement of disposing with, i.e. providing at least one gas station within a radius of maximum 10 km from the headquarters of the Subsidiary Crni Drim" — Ohrid on Str. Zivko Cingo no.8, Ohrid.

Part 4 — Procurement of fuels — Subsidiary "Radovishko Pole” — Radovish

— A list of successfully completed contracts related to the subject of procurement in the last three years, including values of contracts, dates of concluding, buyers (contracting authorities or economic operators), by providing a certificates of performed deliveries issued by recipients, or, if such certificates cannot be provided for reasons beyond the control of the economic operator, only with his statement on performed deliveries

— a statement of disposing with, i.e. providing at least one gas station within a radius of maximum 10 km from the headquarters of the Subsidiary "Polog” on Str. JNA no.139 — Gostivar.

Part 6 — Procurement of fuels — Subsidiary "Kumanovsko Lipkovsko Pole” — Kumanovo

— a statement of disposing with, i.e. providing at least one gas station within a radius of maximum 10 km from the headquarters of the Subsidiary Kumanovsko Lipkovsko Pole, village of Lipkovo (Opae point).

Part 7 — Procurement of fuels — Subsidiary "Berovsko Pole” — Berovo

— a statement of disposing with, i.e. providing at least one gas station within a radius of maximum 10 km from the headquarters of the Subsidiary "Berovsko Pole” on Str. Marshal Tito bb, Berovo. The proof are prepared by the economic operator himself and they should be digitally signed with a digital certificate by the responsible or authorized person of the economic operator.

PART 8 — Procurement of fuels — Subsidiary "Bitolsko Pole” — Bitola

— a statement of disposing with, i.e. providing at least one gas station within a radius of maximum 10 km from the headquarters of the Subsidiary "Bitolsko Pole” on Str. “Prilepska” bb — Bitola.

Part 9 — Procurement of fuels — Subsidiary "Bitolsko Pole” — Bitola

— Proof (Traffic license or lease agreement) that the economic operator owns at least 1 (one) tanker with ADR certificates intended for the transport of dangerous matter, with calibrated flow meter issued by the Bureau of Metrology and other legal entities authorized by the Ministry of Economy to establish the exact delivered quantity of fuel.

Part 10 — Procurement of fuels — Subsidiary "Skopsko Pole” — Skopje

— a statement of disposing with, i.e. providing at least one gas station within a radius of maximum 10 km from the headquarters of the Subsidiary "Skopsko Pole” on Str. Third Macedonian Brigade, no. 10a — Skopje.

The proof are prepared by the economic operator himself and they should be digitally signed with a digital certificate by the responsible or authorized person of the economic operator.

Part 17 — Procurement of fuels — Directorate of JSC Water Management of the Republic of Macedonia in state ownership, Skopje

— a statement on the use of cashless payment system, and

— a Statement of disposing with, i.e. providing at least one gas station on the territory of the city of Skopje, at least one gas station on the territory of the municipalities Ohrid, Bitola, Gevgelija, Shtip, Kumanovo, Veles and Tetovo.

Prilepska bb — Bitola.

Part 10 — Procurement of fuels — Subsidiary ‘Skopsko Pole’ — Skopje — experience with successfully completed contracts related to the subject of procurement in the last three years, — a technical capacity to use a cashless payment system and — disposing with, i.e. providing at least one gas station within a radius of maximum 10 km from the headquarters of Subsidiary ‘Skopsko Pole’ on Str. Third Macedonian Brigade, no. 10a — Skopje.

Part 11 — Procurement of fuels — Subsidiary ‘Skopsko Pole’ — Skopje — experience with successfully completed contracts related to the subject of procurement in the last three years — Technical capacity of the economic operator that he owns at least 1 (one) tanker with ADR certificates intended for the transport of dangerous matter, with calibrated flow meter issued by the Bureau of Metrology or other legal entities authorized by the Ministry of Economy, which will be used to deliver the fuel Euro diesel BS (D-EV) in the village of Jurumleri Skopje.

Leninova no.21 — Strumica.

Part 13 — Procurement of fuels — Subsidiary ‘Bregalnica’ — Kocani — experience with successfully completed contracts related to the subject of procurement in the last three years, — a technical capacity to use a cashless payment system and — disposing with, i.e. providing at least one gas station within a radius of maximum 10 km from the headquarters of Subsidiary ‘Bregalnica’ on the territory of Str. VMRO no.20 — Kocani.
