— Statement of the economic operator that in the past five years it was not rendered a final judgment for participation in criminal organization, corruption, fraud or money laundering
— Confirmation by a competent organ that a…
… bankruptcy procedure was not opened
… liquidation procedure was not opened
— Confirmation for paid taxes, contributions and other public duties by a competent organ in the country where the economic operator was registered
— Certificate from the Registry of penalties for crimes committed by legal persons that minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him
— certificate from the Registry of penalties for crimes committed by legal persons that a minor penalty for temporary or permanent prohibition of a specific activity has not been imposed over him;
— confirmation that a final judgment — prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.
Capacity for performing professional activity of economic operator:
Document for registered activity
1. In order to prove its capacity for performing professional activity, the economic operator should be registered as natural or legal entity for performing the activity related to the subject of public procurement contract or to be a member of a relevant professional association in accordance to the regulations of the country where it is registered. The economic operator proves the meeting of the minimum conditions of point 1 by submitting a document for registered activity issued by the Central Register of the Republic of Macedonia, as an evidence that it is registered as natural or legal entity for performing the activity related to the subject of the public procurement contract or an evidence that it belongs to a relevant professional association in accordance to the regulations of the country where it is registered. 2. An evidence issued by a competent organ for meeting the special conditions for performing the activity prescribed pursuant to a law, which are related to contract subject. The economic operator should be registered for performing activity of wholesale with medical devices. The economic operator shall prove the minimum conditions of point 2 by submitting a valid Decision for wholesale of medical devices on the territory of the Republic of Macedonia issued by the Agency for Medicines and Medical Devices of the Republic of Macedonia.

Valid decision that approves putting into market circulation of medical device (procurement subject –reagents noted in Part 1,2,3,5,6,7,8,9,10 and 11) on the territory of the Republic of Macedonia or approval for parallel import or temporary approval for parallel import for medical device issued by the Agency for Medicines and Medical Devices of the Republic of Macedonia.

The offered goods (procurement subject –reagents noted in Part 1,2,3,5,6,7,8,9,10 and 11) should be approved for putting on the market on the territory of the Republic of Macedonia, i.e. to be registered or approved for parallel import by the Agency for Medicines and Medical Devices of the Republic of Macedonia. The supplies (Part 4,12,13 and 14) are not subject to registration.
