In order to prove its personal circumstances, the economic operator should submit the following documents:

— Statement of the economic operator that, within the last five years, he/she has not had any legally effective judgment for participation in a criminal organization, corruption, fraud or money laundering, pronounced against him/her;

— Certificate that it is not subject to a bankruptcy procedure opened by a competent authority;

— Certificate that it is not subject to a liquidation procedure opened by a competent authority;

— Certificate of paid taxes, contributions and other public dues issued by a competent authority in the country where the economic operator is registered;

— Certificate from the Registry of Offences for Committed Crimes by Legal…

… Persons that a secondary sentence for prohibition of participation on public bidding procedures or awarding of public procurement or public private partnerships contracts has not been issued against it;

… Persons, that a secondary sentence for provisional or permanent prohibition to execute a specific business activity has not been issued against it;

— Certificate that there is no legally effective judgment imposing a misdemeanour sanction involving prohibition to perform a profession, activity or duty, or prohibition to perform a specific activity, imposed against it.

IV.2.2) Capability of the economic operator to perform a professional activity:

— Document on registered economic activity document on registered economic activity as proof that it is registered as a natural or legal persons performing the economic activity related to the subject matter of the public procurement contract or proof that it belongs to an appropriate professional association as per the regulations of the country where it is registered.

Minimum criteria for establishing the technical and professional circumstances of the economic operator:

— Bidder must have at its disposal certified persons for networking, system and virtual environments, as well as certified persons in the field of information security, project management, and e-mail system. The Bidder must have at its disposal as least the following:

— 2 certified persons for virtual environment, in the environment (technology) of the offered solution,

— 2 certified persons — system administrators, for the systems used by the beneficiary, of whom one certified person for system administrator of operating systems, minimum Windows Server 2012, and one certified person, minimum Linux (Ubuntu, RedHat, CentOS),

— 1 certified person in the field of e-mail used by the beneficiary, minimum Exchange Server 2013,

— 1 certified person for databases used by the beneficiary, minimum Microsoft SQL Server 2013,

— 2 certified persons for networks, in the field (technology) of the offered solution,

— 2 certified persons for security (security administrator), in the field (technology) of the offered solution,

— 1 certified person in the field of project management (PMP, Prince or equivalent),

— 1 certified person in the field of process management (ITIL or equivalent),

— 2 certified persons — auditors for information systems security management.

Note: one person may be listed in several roles:

— Supplying licensed software for operating systems, databases, and e-mail system, as an integral part of the hosting service,

— Conducted external audit of the information system and the information infrastructure and manual processing of personal data in the last 3 (three) years by a company authorised for this purposes by the Personal Data Protection Directorate,

— Bidder must have previous experience in implementations and deliveries in the operator's data centre for at least 2 (two) contracts in the last 3 (three) years.

— Statement on engaged technical staff, by enclosing valid certificates for attained know-how, irrespective whether they belong directly to the Economic Operator, either in original or a copy certified by the economic operator for at least:

— 2 certified persons for virtual environment, in the environment (technology) of the offered solution,

— 2 certified persons — system administrators, of whom one certified person for system administrator of operating systems, minimum Windows Server 2012, and one certified person for system administrator in the field of operating systems, minimum Linux (Ubuntu, RedHat, CentOS),

— 1 certified person in the field of e-mail, minimum Exchange Server 2013,

— 1 certified person for databases, minimum Microsoft SQL Server 2013,

— 2 certified persons for networks, in the field (technology) of the offered solution,

— 2 certified persons for security (security administrator), in the field (technology) of the offered solution,

— 1 certified person in the field of project management (PMP, Prince or equivalent),

— 1 certified person in the field of process management (ITIL or equivalent),

— 2 certified persons — auditors for information systems security management (CISA, CIA or equivalent),

— Submit document confirming the right to supply licensed software for operating systems (Microsoft Windows 2012 or equivalent), databases (SQL Server 2013 or equivalent), and e-mail system (Exchange Server 2013 or equivalent), as an integral part of the hosting service,

— Submitting certificate on conducted external audit of the information system and the information infrastructure and manual processing of personal data in the last 3 (three) years by a company authorised for this purposes by the Personal Data Protection Directorate,

— Reference list of delivered services (including values, dates, and contracting authorities) related to the procurement subject matter in the last 3 years, by providing at least 2 (two) certificates for completed deliveries issued by the recipients or if such certificates may not be obtained due to reasons outside the control of the Economic Operator, only a statement by the latter for the completed deliveries.
