In order to prove its personal condition the economic operator is obliged to submit the following documents:

— Statement of the economic operator that in the past five years it was not rendered a final judgment for participation in criminal organization, corruption, fraud or money laundering.

— Confirmation by a competent organ that a…

… bankruptcy procedure was not opened.

… liquidation procedure was not opened.

— Confirmation for paid taxes, contributions and other public duties by a competent organ in the country where the economic operator was registered.

— Certificate from the Registry of penalties for crimes committed by legal persons that minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him.

— certificate from the Registry of penalties for crimes committed by legal persons that a minor penalty for temporary or permanent prohibition of a specific activity has not been imposed over him;

— confirmation that a final judgment — prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.

Capacity for performing professional activity of economic operator:

Document for registered activity.

In order to prove its capacity for performing professional activity, the economic operator should submit a document for registered activity as an evidence that it is registered as natural or legal entity for performing the activity related to the subject of public procurement contract or an evidence that it belongs to a relevant professional association in accordance to the regulations of the country where it is registered. When determining the capacity for performing professional activity of economic operators, the contracting organ shall also request for a an evidence issued by a competent organ for meeting the special conditions for performing the activity prescribed in accordance to law, which are related to the contract subject. In order to prove its capacity for performing professional activity, the economic operator should submit a document for registered activity — DRD Form or confirmation for registered activity. –Decision for wholesale of medical devices pursuant to the Law on Medicines and Medical Devices.

Техничка и професионална способност:

The economic operator shall prove the meeting of minimum conditions by submitting: For registered medical devices -Decision for putting into market circulation of medical device or temporary or permanent approval for parallel import, pursuant to the Law on Medicines and Medical Devices. For unregistered medical devices — Import license (approval for intervention import) pursuant to the Law on Medicines and Medical Devices.

Минимално ниво(а) на стандарди:

In order to qualify itself as capable for performing the subject public procurement contract from the aspect of its technical or professional capacity, the economic operator should meet the following minimum conditions: For registered medical devices -the economic operator, for the offered goods, should provide approval for putting into circulation on the market of medical device or temporary or permanent approval for parallel import, pursuant to Law on Medicines and Medical devices. For unregistered medical devices — the economic operator, for the offered goods, should provide a document allowing import (approval for intervention import) pursuant to Law on Medicines and Medical devices.
