In order to prove its personal status, the economical operator should submit the following documents:
— statement of the economic operator stating that no effective verdict has been pronounced in the past five years for participation in criminal organization, corruption, fraud or money laundering,
— confirmation that no bankruptcy procedure has been initiated by a competent authority,
— confirmation that no liquidation procedure has been initiated by a competent authority,
— confirmation for paid taxes, contributions and other public fees from the authority competent in the country where the economic operator has been registered,
— confirmation from the Registry for penalties for committed criminal acts of the legal entities stating that no secondary penalty,
ban on participation in open call procedures, on awarding of contracts for public procurement and contracts on public-private partnership has been pronounced,
temporary penalty or permanent ban on the performance of particular activity has been pronounced,
— confirmation stating that no misdemeanour sanction — ban on the performance of the profession, activity or obligation, i.e. temporary ban on the performance of a particular activity has been pronounced with the effective verdict.
Capability for performing professional activity of the economical operator:
— Document for registered professional activity (DRA form), as a proof for registration as a physical person or legal entity for performance of the activity related to the subject of the contract on public procurement or a proof of belonging to a related professional association in accordance with the regulations of the country of registration, — ‘A’ Licence for supervision of first category constructions in accordance with the Law on Construction, issued by the Ministry of Transport and Communications, and if the economic operator is foreign bidder, a confirmation is submitted of his capability to perform supervision of first category constructions, issued by a competent authority of the state administration responsible for spatial planning of RM, in accordance with article 42 of the Law on Construction, as proof of meeting the special criteria for performing professional activity prescribed by law, in reference to the subject of the contract.

Економска и финансиска состојба:
The following documents are necessary in order to prove the economic capability of the economic operator:
— a statement of the overall turnover of the company (information on the profit and loss statement issued by the competent authority, i.e. audited profit and loss statement), and where applicable, of the turnover in the area covered by the public procurement contract for the last three business years for which such information is available, depending on the date on which the company was founded or started work and depending on the availability of such information.

Minimum criteria for determining the economic and financial status of the economic operator:
— The economic operator should have a positive financial performance in each of the last three years (2014, 2015 and 2016).
Техничка и професионална способност:
Documents necessary to prove the technical and professional capability of the economic operator:
1) Reference list of completed contracts in the last three years for having completed at least 1 (one) contract for supervision of objects with a value of at least 8,000,000.00 MKD, including data on suppliers, dates and values, attaching at least 1 (one) copy of the contract and 1 (one) certificate for timely and qualitatively executed contract, which refers to the attached contract, or if such certificates can not be provided for reasons beyond the control of the economic operator, only statement on performed services.

2) A statement of the engaged technical staff that will include basic data on the persons engaged, their qualifications and the required authorizations.
3) A statement of the economic operator that he disposes with an accredited laboratory for testing construction materials, attaching a Certificate of accreditation of the laboratory for examination of construction materials MKS EN ISO/IEC 17025 2006 or equivalent, together with accredited methods of examination (scope of accreditation).

Minimum criteria needed for establishing the technical and professional condition of the economic operator:
1) The economic operator to have realized at least 1 (one) contract for the supervision of objects with value of at least 8 000 000 MKD in the last 3 (three) years
2) The economic operator to have at least 14 (fourteen) engaged people, of which: — at least 3 (three) construction engineers, of which 1 (one) with A authorization for supervision and 2 (two) with a B authorization for supervision of constructions — at least 2 (two) engineers architects, of which 1 (one) with A authorization for supervision and 1 (one) with a B authorization for supervision of constructions — at least 2 (two) mechanical engineers, of which 1 (one) with A authorization for supervision and 1 (one) with a B authorization for supervision of constructions — at least 2 (two) electrical engineers, of which 1 (one) with A authorization for supervision and 1 (one) with a B authorization for supervision of constructions — at least 1 (one) engineer geodet and — at least 4 (four) technicians of which 2 (two) construction technicians and 1 (one) electrical technician and 1 (one) architectural technician;

3) The economic operator should dispose with an accredited laboratory for testing construction materials (geomechanics, asphalt and concrete) that, within the scope of accreditation, includes: geomechanics, aggregates, stone and ceramics, asphalt, bitumen and insulating materials, concrete, steel, cement and construction chemistry. The laboratory should have accredited test methods for masonry mortar, bearing the standard MKS EN 1015-4 2009 or equivalent and MKS EN 1015-7 2009 or equivalent, Test methods for masonry units, MKS EN 772-16 2000 or equivalent,MKS EN 772-13 2009 or equivalent, MKS EN 772-20 CH/2009 /A1CH2009 or equivalent, and the Laboratory to have an accreditation certificate MKS EN ISO/IEC 17025 2006 or equivalent.
