— confirmation stating that no liquidation procedure has been initiated by a competent body,

— certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered,

— confirmation from the Register of penalties for committed criminal offenses of legal entities proving that no secondary penalty —…

… ban on participating in open call procedures, awarding public procurement contracts and contracts on public - private partnership has been pronounced,

In the event that the economic operator cannot obtain a confirmation from the service user, then the economic operator shall submit a statement specifying the details of the contract including the value, date and name of the purchaser.

In the event that the economic operator cannot obtain references from the service user, then the economic operator shall submit statements (2) specifying the details of the contract including the values, dates and name of the service user and the number of users of the system for the implemented contracts.

The reference should be issued by the service user with specified values and dates for the implemented contract. In the event that the economic operator cannot obtain a confirmation from the service user, then the economic operator shall submit a statement specifying the details of the contract including the values, dates and name of the service user.

… the position certified person for Oracle Database 11g Administration by submitting M1/M2 form or the relevant contract for the employed person – certified person for Oracle Database 11g Administration, submitting a certificate for Oracle Database 11g Administration (or equivalent) or newer edition and by submitting at least 2 references from projects for development of electronic systems where Oracle Database (or equivalent) was used and where the proposed person took part. The references should be issued by the service user with specified values and dates for the implemented contract. In the event that the economic operator cannot obtain a confirmation from the service user, then the economic operator shall submit statements (2) specifying the details of the contract including the values, dates and name of the service user.

… the position certified person for Oracle Database 11g Performance Tuning by submitting M1/M2 form or the relevant contract for the employed person – certified person for Oracle Database 11g Performance Tuning, submitting a certificate for Oracle Database 11g Performance Tuning (or equivalent) and by submitting at least 2 references from projects for development of electronic systems where Oracle Database (or equivalent) was used and where the proposed person took part. The references should be issued by the service user with specified values and dates for the implemented contract. In the event that the economic operator cannot obtain a confirmation from the service user, then the economic operator shall submit statements (2) specifying the details of the contract including the values, dates and name of the service user.
