In order to prove its personal status, the economical operator should submit the following documents:

— statement of the economic operator stating that no effective verdict has been pronounced in the past five years for participation in criminal organization, corruption, fraud or money laundering,

— confirmation that no bankruptcy procedure has been initiated by a competent authority,

— confirmation that no liquidation procedure has been initiated by a competent authority,

— confirmation for paid taxes, contributions and other public fees from the authority competent in the country where the economic operator has been registered,

— confirmation from the Registry for penalties for committed criminal acts of the legal entities stating that no secondary penalty,…

… ban on participation in open call procedures, on awarding of contracts for public procurement and contracts on public-private partnership has been pronounced,

… temporary penalty or permanent ban on the performance of particular activity has been pronounced,

— confirmation stating that no misdemeanour sanction – ban on the performance of the profession, activity or obligation, i.e. temporary ban on the performance of a particular activity has been pronounced with the effective verdict.

Capability for performing professional activity of the economical operator:

A document for registered professional activity.

For part 1, part 2 and part 3: A document for registered professional activity as a proof for registration as a physical person or legal entity for performance of the activity related to the subject of the contract on public procurement or a proof of belonging to a related professional association in accordance with the regulations of the country of registration.

For part 1: Work certificate for performing specialized services with an aircraft, on the name of the bidder (with a valid deadline), which, among other things, regulates the basic condition for performing such type of activity i.e., the disposal of chemical means and water from an aircraft (Destruction of insects in agriculture; Destruction of animal pests), issued by the Civil Aviation Agency of the Republic of Macedonia, and based on the Aviation Law of the Republic of Macedonia (Official Gazette of the Republic of Macedonia no. 14/06; 24/07; 103/08; 67/10; 24/12; 42/14; 95/15 and 152/15), as well as the conditions from Rulebook on the manner of performing specialized services with an aircraft and the special requirements in respect of the aircrafts, equipment and other special requirements necessary for safe and proper operation (Official Gazette of the Republic of Macedonia no. 159/07 and 122/13);

For part 3: Decision for performing the wholesale trade with chemicals, on the name of the bidder (with a valid deadline), issued by the Ministry of Health on the basis of the Law on Chemicals (Official Gazette of the Republic of Macedonia no. 145/10, 53/11 and 164/13) as well as the provisions of the Rulebook on the detailed conditions regarding the space, equipment and personnel to be fulfilled by the legal entities that perform wholesale trade with chemicals (Official Gazette of the Republic of Macedonia no. 132/08).
