In order to prove its personal condition the economic operator is obliged to submit the following documents:
— Statement of the economic operator that in the past 5 years it was not rendered a final judgment for participation in criminal organization, corruption, fraud or money laundering,
— Confirmation by a competent organ that a…
… bankruptcy procedure was not opened,
… liquidation procedure was not opened,
— Confirmation for paid taxes, contributions and other public duties by a competent organ in the country where the economic operator was registered,
— Certificate from the Registry of penalties for crimes committed by legal persons that minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him,
— certificate from the Registry of penalties for crimes committed by legal persons that a minor penalty for temporary or permanent prohibition of a specific activity has not been imposed over him,
— confirmation that a final judgment — prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.
Capacity for performing professional activity of economic operator:
Document for registered activity.
In order to prove its capacity for performing professional activity, the economic operator should be registered as natural or legal entity for performing the activity related to the public procurement subject or to be a member of a relevant professional association in accordance to the regulations of the country where it is registered. The economic operator shall prove the fulfillment of the minimum conditions of the above stated point by submitting a document for registered activity issued by the Central Register of the Republic of Macedonia, as an evidence that it has been registered as legal or natural entity for performing the activity related to the public procurement subject or an evidence that it is a member of a relevant professional association in accordance to the regulations of the country where it is registered. Evidence issued by a competent organ for meeting the special conditions for performing the activity prescribed according to law, which refers to the subject of the contract. The economic operator should be registered for performing activity — wholesale of medical devices. The economic operator shall prove the fulfillment of the minimum conditions of the previous point by submitting a valid Decision for wholesale of medical devices on the territory of the Republic of Macedonia issued by the agency for medicines and medical devices of the Republic of Macedonia.

Техничка и професионална способност:
The economic operator shall prove the fulfillment of the minimum conditions by submitting of:
1. For registered medical devices — Decision for putting into market circulation of medical device or temporary or permanent approval for parallel import, pursuant to the law on medicines and medical sevices.
2. For unregistered medical devices — Import license (approval for intervention import) pursuant to the law on medicines and medical devices.
3. Statement by engaged professional technical personnel and technical bodies obliged to assemble, maintain and service the equipment, regardless of the fact if they are immediately part of the economic operator or not.
Минимално ниво(а) на стандарди:
In order to qualify as capable of performing the subject public procurement contract from the aspect of its technical or professional capacity, the economic operator should meet the following minimum conditions:
1. For registered medical devices — for the offered goods, the economic operator should provide approval for putting into circulation on the market of medical device or temporary or permanent approval for parallel import, pursuant to law on medicines and medical devices.

2. For unregistered medical devices — the economic operator, for the offered goods, should provide a document allowing import (approval for intervention import) pursuant to law on medicines and medical devices.
3. The economic operator should have professional technical personnel and technical bodies obliged to assemble, maintain and service the equipment.
