Information and formalities necessary for evaluating if the requirements are met:

That in the last 5 years he has an effective sentence for participation in a criminal organization, corruption, fraud or money laundering;

— who has been stated ban on participation in public bidding procedures, award of contracts and agreements for public-private partnerships,

— which was announced a minor penalty temporary or permanent prohibition of professional activity,

— which is in bankruptcy or liquidation proceedings,

— who has unpaid taxes and other public charges,unless the economic operator has been granted deferred payment of taxes,contributions or other taxes in accordance whit special regulations and regularly paid,

— was pronounced a sanction -ban on performing profession, activity or duty, or prohibition on specific activity and providing false information or fails to submit the information requested by the Contracting Authority.

Documents for registered activity as evidence that it is registered as a legal entity for performing the activity related to the subject of the contract or proof that they belong to a relevant professional association in accordance with the regulations of the country which grants approval.

Економска и финансиска состојба:

— have positive results in working for the last 3 (three) years,

— have an income not less than the estimated value of the procurement in at least one of the last 3 years,

— 2.4.2 If the economic operator performs the activity for a period of less than 3 years, the minimum condition for determining the economic and financial condition is that the economic operator has fulfilled the requirement in Subsection 2.4.1 in the years for which data exist.

Минимално ниво(а) на стандарди:

A statement of the total turnover of the enterprise (data from the profit and loss account issued by a competent authority, ie revised profit and loss account) and, where appropriate, from the turnover in the area covered by the public procurement contract, mostly for the last 3 financial years for which such information is available, depending on the date on which the enterprise was established or started operating, and depending on the availability of such information.

Техничка и професионална способност:

At least 6 persons with their name and surname that will be engaged by the economic operator for the realization of the subject procurement, with work experience of at least 2 years in the adaptation, implementation and maintenance of hospital information systems, and:

— 2 (two) developers,

— 1 (one) database administrator,

— 1 (one) system administrator,

— 1 (one) business analyst,

— 1 (one) authorized accountant (required for implementation of financial modules).

All team members should be employed by the bidder at least 2 g are alone before the day the bid is submitted. Have at least 2 successful implementations of an integrated hospital information system in the last 2 years in the Republic of Macedonia. Macedonia:

— to have at least 4 people in permanent employment and to actively use a helpless system for registering questions / requests and communication with users,

— to have user manuals for the system, for all types of users, in which the required functionalities can be seen of the technical specification Proof of relevant insurance for occupational risk compensation.

Минимално ниво(а) на стандарди:

A list of at least 6 members of the team working on the project, a role that each member has in the team, and a description of the skills and professional experience each member of the team has:

— statement from the responsible person that all members of the team working on the project are employed by the bidder, in the requested period, full time and at least 2 (two) developers:

—— at least 1 (one) database administrator;

—— at least 1 (one) system administrator;

—— at least 1 (one) business analyst about at least 1 (one) person with licensed certified accountant Attachment М1/М2 for all listed persons and a copy of a licensed certified accountant for at least 1 person;

— copies of certificates and/or transcripts for at least 2 successfully implemented RTD hospital information systems PHI in R. Macedonia, similar in functionality of the offered system,

— statement for engaged staff and statement for use of a helpless system for recording of requests / requests and communication with users for providing support of systems similar to the offered, and Annex M1/M2 for the mentioned staff,

— attachment: user manuals for the system for each user profile,

— as proof of relevant insurance for professional risk insurance, the economic operator encloses a copy of the concluded contract with a competent legal entity in the Republic of Macedonia and a copy of the statements for settlement of the contractual obligations.
