— certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered,

— confirmation from the register of penalties for committed criminal offenses of legal entities proving that no secondary penalty —…

… ban on participating in open call procedures, awarding public procurement contracts and contracts on public/private partnership has been pronounced,

… temporary or permanent ban on performing particular activity has been pronounced;

— confirmation stating that no misdemeanour sanction — ban on the performance of the profession, activity or duty, i.e. temporary ban on the performance of a particular activity has been pronounced with an effective verdict.

— In order to determine the legal requirement — declaration of independent bid to be signed by the authorized person and it will be verified if the bid is digitally signed by the authorized person. The bidder should also provide current state issued by the Central Registry of the Republic Macedonia.

— Statement that he will provide a training to the staff by an authorized trainer and issue a certificate for the passed training (these statements are not signed manually, but electronically by attaching a valid digital certificate whose bearer is the authorized person or a person authorized by him),

— Certificate issued by the manufacturer of the offered instrument,

– Certificate that the bidder is authorized for servicing the offered instrument on the territory of the Republic of Macedonia.

Standards for quality systems:

Part 1: – Submit a quality system standard document i.e. a certified copy of the quality management document ISO9001C2008. The economic operator will prove the fulfillment of the quality with the quality management document ISO9001C2008.

The statement may not be delivered after the due date for receipt of requests pursuant to article 129 paragraph 1 of the Law on Public Procurement. The independent bid statement may not be signed by any other person other than the authorized person. The independent bid statement should be electronically signed by attaching valid digital certificate of the authorized person. If the form is signed by a person authorized to digitally sign documents, then it should be personally signed by the authorized person, scanned and on that for the digital signature should be applied.
