Personal situation of economic operators, including requirements relating to enrollment on professional or trade registers:

— in the last 5 years there was an effective conviction for participation in criminal organization, corruption, fraud or money laundering,

— there is minor penalty that prohibits participation in procedures for public announcement, awarding contracts on public procurements and contract for public-private partnership,

— there is effective conviction for temporary or permanent prohibition to perform certain activity,

— has initiated open bankruptcy procedure or liquidation procedure,

— has unpaid taxes, contributions and other public fees, except if the economic operator has deferred payments for those taxes, contributions and other public fees in accordance with the special regulations and the same are regularly paid,

— there is effective conviction by which infringement sanction has been imposed on — interdiction for professional activities, activities or duties, i.e. temporary interdiction of separate activity realization and,

— provides false information or does not submit the information requested by the Contracting Authority.

— Document for registered activity as proof that it is registered as a legal entity for performing the activity related to the subject of the contract or proof that they belong to a professional association in accordance with the regulations of the country where it is registered.

For proving personal standing, the economic operator submits the following documents:

— statement of economic operator that in the last 5 years there was no effective conviction for participation in criminal organization, corruption, fraud or money laundering,

— certificate that any competent authority has initiated no open…

… bankruptcy procedure,

… liquidation procedure,

— certificate for paid taxes, contributions and other public payments by competent authority in the country where the economic operator has been registered in,

— certificate from the Register of penalties for crimes committed by legal persons that there is no minor penalty that prohibits participation in procedures for public announcement, awarding contracts on public procurements and contract for public-private partnership,

— certificate from Register of penalties for crimes committed by legal persons that there is no effective conviction for temporary or permanent prohibition to perform certain activity,

— certificate that there is no effective conviction by which infringement sanction has been imposed on — interdiction for professional activities, activities or duties, i.e. temporary interdiction of separate activity realization.

— Decision for wholesale of medical supplies under the Law on Medicines and Medical Devices.

Техничка и професионална способност:

— statement of the warranty period for the goods that are subject to the procurement of at least 12 months from the moment of signing the contract – must be used the attached form of a statement in this tender documentation,

— statement for provided maintenance service and repairs of the goods that are the subject of procurement within the warranty period – it is required to be used the attached form of a statement from this tender documentation

— For the offered goods to be provided warranty period of at least 12 months from the moment of signing the contract.

— For the offered goods to be provided maintenance service and repairs within the warranty period.
