To prove its state, the economic operator should submit the following documentation: economic operator's statement that in the last 5 years there was no legal procedure or charges against him for corruption, illegal ventures, fraud or money laundry.

— certificate for paid taxes, incomes, contributions and other types of duties issued by authorities from the state where the economic operator is registered,

— certificate issued by the register for criminal records and charges that no prohibition or…

… verdict have been pronounced to participate in invitation of tenders, to conclude public procurement contracta and to conclude contracts for public-private partnership,

… verdict, permanent or temporary, have been pronounced to perform any activity,

— certificate that there is no verdict or sanction — prohibition to do any kind of job, work, activity or obligation, temporary or permanent.

Documents for registered activities

— the economic operator should be owner of the source software code that is being offered and it ought to have a license for distribution on the territory of the Republic North Macedonia and to prove that by enclosing the appropriate document for the offer.

Documents needed to prove the economical state of the economic operator:

— statement of the total company turnover (data from the profit and loss account issued by authority, or audit from the profit and loss account) and where is suitable turnover of the area which is covered by the public procurement contract mostly for the last 3 financial years with available data depending on the date when the company was established or started to work, and depending on the data availability.

Минимално ниво(а) на стандарди:

Minimal criteria for establishing the economic and financial state of the economic operator:

— to have positive work results during the last (3) three years,

— to have income not less than the estimated purchase value at least in one of the last 3 years.

— 2.4.2 in case the economic operator functions in period less than 3 years, the minimal criteria for revising the economic and financial state is the term that the economic operator had met the condition stated in point 2.4.1. during the years for which data is available.

Техничка и професионална способност:

Necessary documents for establishing the technical and professional ability of the economic operator:

List of minimum 5 team members that shall work on the project, their individual role in the team and description of the skills and professional experience of each team member.

Statement given by authorized person that all the team members that are going to work on the project are employed in bidding company in period of minimum 2 years with full-time work:

— at least (2) two programmers,

— at least (1) one data base administrator,

— at least (1) one business analyst,

— at least (1) one licensed accountant.

Enclosed documents M1M2 for labour and social insurance for each stated worker and copy of the authorized accountant's license for at least (1) person. Certificate copies or/and minutes for at least (3) successfully implemented hospital information systems in the past (3) three years in public health hospitals in the Republic of North Macedonia, similar in system function and content subject of the purchase.

Statement for engaged personnel for support, at least 3 people with minimum 2 years work experience for the economic operator in the purchase area and statement for used helpdesk system for registration of questions and answers (communication) with users in order to provide support to systems similar to the offered. M1M2 forms to be enclosed for each person from the support team.

Annex — user manuals for all system modules and for each user's profile.

Минимално ниво(а) на стандарди:

To provide technical team with qualified members.

— at least (5) five people with name and surname that shall be engaged by the economic operator to perform the purchase, with minimum work experience for the operator during 2 years of development, adaptation and implementation of Integrated web-based hospital informational systems — like the one in the public procurement specifically:

— (2) two programmers,

— (1) one data base administrator,

— (1) one business analyst,

— (1) one licensed accountant (necessary for the financial modules implementation).

All the team members have been employed at least (2) two years before the date of procurement enclosure. There ought to be at least (3) three successful hospital systems implementations similar or same as the subject of this procurement — integrated web-based hospital informational systems for registration of medical and administrative procedures in public health institutions, hospitals in the last (3) three years in the Republic North Macedonia. There ought to be at least (3) three people of the support team who are full-time employed with minimum experience of 2 years in this area that actively use helpdesk system for registration of applications/questions and answers with users. There ought to be user's manual for the system, for all types of users where the function of the technical specifications can be seen.
