— minimum dioptric span from +10 to + 30 with increments 0,5 diopters and all other with increments of 1 diopters — square edge design — Angulation 0 degrees — to be delivered with suitable cartridges for single or multiple usage, for size of cut from 2,6 mm to 3 mm.

The operator shall submit the following documents for evaluation of its capacity:

— a declaration by the economic operator that in the last 5 years he wasn’t rendered a verdict for participation in a criminal organization, corruption, fraud or money laundering,

— confirmation that there are no bankruptcy proceedings opened against the operator by a competent authority,

— confirmation that there are no liquidation proceedings opened against the operator by a competent authority,

— receipt for paid taxes, contributions and other public duties by the competent authority in the country where the operator is registered,

— certificate from the registry of penalties for criminal acts by legal entities that minor penalty…

… for temporary or permanent prohibition of professional activity wasn’t announced,
