— confirmation stating that no bankruptcy procedure has been initiated by a competent body,

— confirmation stating that no liquidation procedure has been initiated by a competent body,

— certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered,

— confirmation from the register of penalties for committed criminal offenses of legal entities proving that no secondary penalty —…

… ban on participating in open call procedures, awarding public procurement contracts and contracts on public — private partnership has been pronounced,

… temporary or permanent ban on performing particular activity has been pronounced,

— confirmation stating that no misdemeanour sanction — ban on the performance of the profession, activity or duty, i.e. temporary ban on the performance of a particular activity has been pronounced with an effective verdict,

— document for registered activity: proof that the bidder is registered as a natural person or a legal entity for performing the activity related to the public procurement subject or a proof that the bidder is a member of a professional association in accordance with the regulations of the country where the bidder is registered.

Техничка и професионална способност:

— list of main deliveries of goods or services performed in the last 3 years, including values, dates, buyers (Contracting Authorities or economic operators), by providing confirmation for deliveries made or services realised,

— reference list with confirmation from the companies for successfully realized contracts (deliveries) of the same type, as the subject of the public procurement that will state as references for realized activity,

— samples, description and/or photos of products that are subject of delivery, and the economic operator is obliged to prove their reliability if required by the Contracting Authority:

— original brochures by the manufacturer of the equipment offers including correct specifications and clearly marked offered characteristics for the products that are be delivered,

— certificates and reports issued by competent bodies for quality control with acknowledged competence, and that test the compliance of the products which was clearly set in the references of the specifications and standards,

— authorisation by the manufacturer (MAF Manufacturer Authorization Form) for the subject public procurement, which the Contracting authority will additionally check at the manufacturer of the equipment,

— standards for quality systems:

—— ISO 9001:2015 certificate (quality management system) or equivalent, issued by accredited body, including number, date of issuance and current validity of the certificate,

—— ISO/IEC 20000-1:2011 certificate (IT service management) or equivalent, issued by accredited body, including number, date of issuance and current validity of the certificate,

—— ISO 27000-1:2013 certificate (information security management) or equivalent, issued by accredited body, including number, date of issuance and current validity of the certificate.

— standards for environmental management: ISO 14001 : 2015 certificate (environmental management) or equivalent, issued by accredited body, including number, date of issuance and current validity of the certificate.
