For Lot 4: a decision on approval of food operators and facility for production, processing and handling of food of animal origin:

— statement that in the last 5 years no effective court verdict has been pronounced for committed criminal act from Article 88 paragraph 1 of the PPL,

— certificate that it is not subject to bankruptcy procedure issued by competent authority,

— certificate that it is not subject to liquidation procedure issued by competent authority,

— certificate for paid taxes, contributions and other public duties by the competent authority in the country where that economic operator is established,

— certificate from the Legal Entities Penal Registry that it is not being imposed a side sanction —…

… prohibition to participate in public competitions, contract award procedures and PPP award procedures,

… temporary or permanent prohibition to perform certain professional activity,

— certificate that it is not being sanctioned for misdemeanour with an effective court decision, resulting in prohibition for pursuing professional activity or duty, i.e. temporary prohibition for performing professional activity.

Техничка и професионална способност:

With list of main deliveries or services performed in the last 3 years, including the values, date of signing, buyers (contracting authorities or economic operators), by providing a confirmation letter of performed deliveries or services: a list of successfully realised contracts related to the subject of procurement in the last 3 years, including the value of the contracts, date of signing, buyers (contracting authorities or economic operators), by providing a confirmation letter of performed deliveries issued by the recipients or, if such confirmation letters may not be obtained due to reasons non-controlled by the economic operator, statement of performed delivery may be submitted only:

— with description of the technical staff and the technical bodies that will participate in the performance of the public procurement, especially of those responsible for quality control and in case of public procurement of works, those included in performance of the works, irrespective of whether they are employees of the economic operator or not: statement for hired technical staff and technical bodies irrespective of whether they belong to the economic operator or not.

With statement of the instruments, plants or technical equipment available to the service provider or the works contractor for executing the contract: statement of availability i.e. provision of minimum 1 (one) freight vehicle for distribution of the food products, while for the distribution of the food (meat, eggs, fish, milk and dairy products), minimum 1 (one) transport vehicle with cooling chamber.
