In order to prove its personal status, the economic operator should submit the following documents:

— statement of the economic operator that no effective verdict has been issued against the economic operator within the past five years for participation in a crime organization, corruption, fraud or money laundering,

— confirmation stating that no bankruptcy procedure has been initiated by a competent body,

— confirmation stating that no liquidation procedure has been initiated by a competent body,

— certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered,

— confirmation from the register of penalties for committed criminal offenses of legal entities proving that no secondary penalty —…

… ban on participating in open call procedures, awarding public procurement contracts and contracts on public — private partnership has been pronounced,

… temporary or permanent ban on performing particular activity has been pronounced,

— confirmation stating that no misdemeanour sanction — ban on the performance of the profession, activity or duty, i.e. temporary ban on the performance of a particular activity has been pronounced with an effective verdict.

Capability of the economic operator for performing professional activity:

— document for registered activity for lot 1, lot 2 and lot 3,

— document for registered activity as a proof that it is registered as natural person or legal entity for performing the activity related to the subject of the public procurement contract or proof that it belongs to proper professional association pursuant to the regulations of the country it is registered.

For Lot 1: certificate for performing specialised services by airplane, on the name of the bidder (valid) which among others regulates as well the basic requirement for performing this type of business activity that is throwing chemical disinfectants and water from the airplane (Extirpation of insects in agriculture; Disinfection of animal pests), issued by the Civil Aviation Agency of RM, and based on the law on aviation of RM (Official Gazette of RM No. 14/06, 24/07, 103/08, 67/10, 24/12, 42/14, 95/15 and 152/15), as well as the conditions from the Rulebooks on the manner of performing specialised services by airplane and the special conditions regarding the airplanes, equipment and other special conditions necessary for safety and prompt work (Official Gazette of RM No. 159/07 and 122/13);

For Lot 3: decision on performing the activity wholesale trade of chemicals, on the name of the bidder (valid), issued by the Ministry of Health, and based on the law on chemicals (Official Gazette of RM No. 145/10, 53/11 and 164/13) as well as the provisions from the rulebook on the closer conditions regarding the space, equipment and staff the legal entities performing the wholesale trade with chemicals should fulfil (Official Gazette of RM No. 132/08).

Техничка и професионална способност:

For Lot 3 reference list for performed deliveries in the last 3 years, stating the data for name of the supplier, description of the delivery, date of the delivery performed, title of the disinfectants delivered, by proving minimum 6 (six) confirmations for performed deliveries of the disinfectants the bidder has offered in this procedure for awarding public procurement contracts (minimum 3 of which relate to delivery of disinfectant for adult forms of mosquitos and minimum 3 confirmations relate to deliveries of disinfectant for mosquito larva), issued by the receivers or if such confirmations cannot be obtained by reasons out of control of the economic operators only by its statement for performed deliveries.

Note: If the Contracting Authority has doubt in the given data and documents delivered for proving the technical or professional capacity, it may additionally check the same.

Минимално ниво(а) на стандарди:

For Lot 3 the economic operator shall have at least 3 (three) deliveries of disinfectant (preparation) for adult forms of mosquitos (disinfection) the bidder has offered in this procedure for awarding public procurement contract and at least 3 (three) deliveries of disinfectant (preparation) for mosquito larva (disinfection) that the bidder has offered in this procedure for awarding public procurement contract in Republic of Macedonia in the last 3 (three) years.
