Краток опис:

The supplier is obliged to deliver the subject of the agreement for public procurement once in a period not longer than 60 days from the day when both parties sign the agreement, to the following location: Public Health Institution Specialized hospital for prevention, treatment and rehabilitation of cardiovascular diseases – Ohrid, settlement ‘St Stefan’ b.b.

The delivery of the medical equipment is considered completed when the person authorized by the purchaser and the person authorized by the supplier sign an appropriate document.

The supplier is obliged to submit a user’s guidelines in Macedonian or English for the delivered medical equipment.

Способност за вршење на стручната дејност:

A certificate for registered activity (form DRD) as a proof that it is registered as a physical or legal entity for performing the activity related to the subject of the agreement for public procurement or a proof that it belongs to an appropriate professional association in accordance with the regulations in the country where it is registered. In the Republic of North Macedonia the competent authority for issuing this document is the Central Registry of the Republic of North Macedonia.

Минимално ниво(а) на стандарди:

— copies from valid certificates for fulfilling the ISO 9001 and ISO 14001 standards that are related to the economic operator, or another acceptable standard for medical equipment issued by a competent certified body,

— copies from the certificate which proves that the apparatus meets all the requirements for a complete system for managing the quality for designing and production of medical devices in accordance to the standard version ISO 13485:2003 or ISO 13485:2016, or another acceptable standard for medical equipment issued by a competent certified body,

— a statement from the economic operator that the apparatus is new and it is not used before,

— a statement of the economic operator that it will provide…

… guarantee for the equipment, of at least 2 years for the ultrasound machine since it has began functioning and 1 year for the cardiac, transesophageal and linear probe, and in that period the economic operator is obliged to service and replace the spare parts of the medical equipment free of charge,

… spare parts and service for servicing the machine in a post-guarantee period of 7 (seven) years,

— the economic operator shall have at least 3 regularly employed service engineers and to prove that by submitting copies of M1/M2, and

— along with the offer, the economic operator shall submit catalogues/brochures/technical data sheet, etc- with technical characteristics of the subject of agreement. Due to the correct identification of the characteristics of the offered equipment, the technical characteristics of the offered goods which are requested in the technical specifications of the tender documentation shall be clearly marked with ordinal numbers in those materials.

Извршување на договорот
Услови за извршување на договорот:

— a condition for inducing the realization of the agreement with the most favourable elected bidder is providing bank guarantee for quality and promptly fulfillment of the agreement in the amount of 10 % of the agreement value with VAT, which is submitted in its original form in, with an issuing date not longer than 7 days from the signing of the agreement.
