In order to prove its personal condition the economic operator is obliged to submit the following documents:

— statement of the economic operator that in the past 5 years it was not rendered a final judgment for committed crime pursuant to Article 88 paragraph 1 of the public procurement law,

— confirmation by a competent organ that a…

… bankruptcy procedure was not opened,

… liquidation procedure was not opened,

— confirmation for paid taxes, contributions and other public duties by a competent organ in the country where the economic operator was registered,

— certificate from the registry of penalties for crimes committed by legal persons that…

… minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him,

… a minor penalty for temporary or permanent prohibition of a specific activity has not been imposed over him,

— confirmation that a final judgment — prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.

Capacity for performing professional activity of economic operator:

— document for registered activity: confirmation for registered activity (DRD form) as an evidence that it is registered as natural or legal entity for performing the activity related to the public procurement contract subject or an evidence that it is a member of a relevant professional association in accordance to the regulations of the country where it was registered.

Balance sheet report certified by a competent organ, i.e. a revised balance sheet, or abstracts from the balance sheet report in cases when its publication is prescribed by law in the country where the economic operator is registered.

Abstract from the company’s overall turnover (data from the income statement issued by competent organ, i.e. a revised income statement) and where relevant, from the turnover in the field covered by the public procurement contract, at most for the last 3 financial years for which such data are available, depending on the date when the company was established or started its operation and depending on the availability of such data.

List of main deliveries for financial leasing in the last 3 years.

Signed contract with the vehicles supplier, which should be attached to the bid.

Minimum 5 confirmations for provided services issued by the leasing beneficiaries.

Catalogue of products that are subject of delivery (photographs and detailed description of the delivery subject).
