In order to prove its personal condition the economic operator is obliged to submit the following documents:

— statement of the economic operator that in the past 5 years it was not rendered a final judgement for participation in criminal organisation, corruption, fraud or money laundering,

— confirmation by a competent organ that a…

… bankruptcy procedure was not opened,

… liquidation procedure was not opened,

— confirmation for paid taxes, contributions and other public duties by a competent organ in the country where the economic operator was registered,

— certificate from the Registry of penalties for crimes committed by legal persons that…

… minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him,

… a minor penalty for temporary or permanent prohibition of a specific activity has not been imposed over him,

— confirmation that a final judgement — prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.

Capacity for performing professional activity of economic operator:

Document for registered activity – DRD Form (Code 9.609 or equivalent).

The Economic operator should own and submit QMS – Quality Management System Certificate ISO 9001 or equivalent. The Economic operator should also own and submit OHSAS 18001 or equivalent, Occupational Health and Safety Management System Standard.

The Economic operator should also own and submit ISO 14001 Certificate or equivalent – Environment management Standard.

Економска и финансиска состојба:

1) Income balance and balance sheet confirming that annual turnover of minimum 40 000 000,00 MKD was realised in the last 3 (three) years. Confirmation or copy of a concluded contract that proves that services were rendered of the same type as the procurement subject in the past period of 3 (three) years (for each year separately, i.e. 2015, 2016 and 2017), with value of minimum 3 000 000,00 each.

Минимално ниво(а) на стандарди:

1) Total annual turnover of minimum 40 000 000,00 MKD realised in the last 3 (three) years;

2) To have rendered services of the same type as the procurement subject in the past period of 3 (three) years (for each year separately, i.e. 2015, 2016 and 2017), with value of minimum 3 000 000,00 each.

Техничка и професионална способност:

Necessary documents to prove the technical or professional capacity of the economic operator:

1) M1/M2 Forms or confirmations from AVRM (Employment Agency of RM) that it has minimum 200 employees;

2) Copy of minimum 5 (five) certificates — confirmations for attended trainings for occupational safety and health, practical level;

3) Copy of minimum 5 (five) certificates – confirmations for attended trainings for occupational safety and health, theoretical level;

4) Minimum 3 (three) confirmations for quality performed activity – hospitals;

5) Statement that it has at disposal the necessary equipment and adequate machines for general and current cleaning of the facility, with detailed list;

6) Copy of the traffic books for the vehicles that it has at disposal;

7) Confirmations-certificates issued by the Public Health Institute of RM that the cleaning agents used are environment-friendly and confirmation-certificate that the disinfection agents to be used are meet the prescribed norms and standards for their use.

Минимално ниво(а) на стандарди:

1) To have professionally engaged minimum of 200 (two hundred) persons, personnel for providing the services stipulated in the public procurement;

2) Minimum 5 (five) certificates – confirmations for attended trainings for occupational safety and health, practical level;

3) Copy of minimum 5 (five) certificates — confirmations for attended trainings for occupational safety and health, theoretical level;

4) Minimum 3 (three) confirmations for quality performed activity – hospitals;

5) To have at disposal the necessary equipment and adequate machines for general and current cleaning of the facility. List of equipment that should contain minimum the following devices and machines:

— minimum 4 battery-charged machines for cleaning hard floors,

— minimum 17 vacuum cleaners with HEPA filters,

— minimum 1 pump for water extraction,

— minimum 2 sets equipment for alpine descending,

— minimum 1 scaffolding for working on minimum height of 6 meters,

— minimum 2 stairs, certified for working height of up to 9 meters,

— minimum one commodity van,

— minimum 5 vehicles of pick-up type for transport of employees and materials;

6) It should use professional and environment-friendly cleaning agents.
