1) So as to prove personal status, the economic operator should provide the following documents:

— statement by the economic operator that in the previous 5 years, an effective court decision for participation in criminal organisation, corruption, fraud or money laundering, has not been announced,

— a Certificate that it is not subject to bankruptcy procedure issued by a competent authority,

— a Certificate that it is not subject to liquidation procedure issued by a competent authority,

— a Certificate for paid taxes, contributions and other public duties by the competent authority in the country where that economic operator is established,

— a Certificate from the Register of penalties for criminal acts committed by legal entities that a secondary penalty…

… ban on participation in procedures for public announcement, the award of public supply contracts and public-private partnership, has not been imposed,

… temporary or permanent prohibition of certain activities has not been imposed and,

— a Certificate that a sanction for misdemeanour with an effective court decision, resulting in prohibition for pursuing professional activity or duty, i.e. temporary prohibition for performing professional activity, it is not being announced;

2) Document for registered activity — A document for a registered activity as proof that the operator is registered as a natural person or a legal entity to perform an activity related to the public procurement contract or proof of membership in an appropriate professional association in accordance with the regulations of the country where the operator is registered, economic operator will prove the fulfilment of special requirements for performing activity prescribed in accordance with the law by submitting the following document:

— valid license for performing insurance activities in the respective classes of insurance that is subject to public procurement, issued by a competent authority in accordance with the Law on Insurance Supervision

(“Official Gazette of the Republic of Macedonia” No. 27/02, 84/02, 98/02, 33/04, 88/05, 79/07, 8/08, 88/08, 56/09, 67/10, 44/11, 7/12, 30/12, 45/12, 64/12, 23/13,188/13, 30/14, 43/14, 112/14, 153/15, 192/15, 23/16, 83/18 and 198/18):

— for Lot No. 1 for classes of insurance 8, 9 and 13,

— for Lot No.2 — classes of insurance 3 and 10.

Required documents for economic and financial ability of the economic operator:

The economic operator will prove the fulfilment of the minimum requirements by submitting the following documents:

— report from the Insurance Supervision Agency of the Republic of Macedonia for 2017,

— audited financial statements for 2017.

Минимално ниво(а) на стандарди:

For Lot No. 1:

— the economic operator has had realised minimum 600 000 000,00 MKD gross written premium for 2017 (in case the bid refers only to Lot No. 1 or to both Lots),

— to have a positive financial results from the operation in 2017.

For Lot No. 2:

— the economic operator has had realised minimum 200 000 000,00 MKD gross written premium for 2017 (in case the bid refers only to Lot No. 2)

— to have positive financial results from the operation in 2017.

Техничка и професионална способност:

Required documents for technical or professional ability of the economic operator:

For Lot No. 1:

— evidence from a competent authority or a rating agency for the rating of the…

… economic operator,

… reinsurance company,

— copy of the reinsurance contract with the first reinsurer with a minimum “A” credit rating determined by Standard and Poor's or equivalent with validity for 2019 (for foreign acts a translation from authorised court interpreter is needed),

— reinsurance program for 2019,

— providing “Insurance Conditions” (General and Specific) for the concrete procurement,

— a list of provided property insurance services to legal entities in the last 3 years, with values, dates, buyers (contracting authorities and economic operators), and providing of:

— copy of at least three (3) contracts for insurance of property and movables of legal entities for each calendar year in the previous 3 years, with an individual value of the contracts not less than 3 000 000,00 MKD,

— at least three (3) statements for the provided insurance services issued by the recipients, separate for each calendar year in the previous 3 years,

— statement for the amount of the highest paid damages in the last 3 years, per year (2016, 2017, 2018),

— a statement that the economic operator has engaged professional personnel qualified to assess the damages adequate to the concrete procurement,

— notification on the elements of the contract which the economic operator intends to subcontract to third parties (if necessary).

For Lot No. 2:

— copy of the reinsurance contract with the first reinsurer with a minimum “A -” credit rating determined by Standard and Poor's or equivalent with validity for 2019 (for foreign acts a translation from authorised court interpreter is needed),

— copy of at least three (3) contracts for insurance of motor vehicles of legal entities (from automobile liability for damages caused to third parties and Full Motor Vehicle Casco Insurance)

For each calendar year in the previous 3 years, with an individual value of the contracts not less than 200 000,00 MKD,

Минимално ниво(а) на стандарди:

— the economic operator to have a minimum "A" credit rating set by Standard & Poor's or equivalent,

— 100 % reinsurance of the stocks of the compulsory oil reserves subject to the public procurement contract,

— having reinsurance coverage from a reputable reinsurance company which has a minimum credit rating of "A" fixed by Standard and Poor's or equivalent, if the amount of insurance for the particular subject of public procurement is greater than the amount of the maximum self-insured retention determined by the Reinsurance Program of the insurance company,

— in the last 3 years having performed an insurance services for the same or similar risks for at least 3 legal entities,

— having engaged (with an employment contract or other agreement) professional personnel qualified to assess the damages of the respective public procurement.

— the economic operator to have a minimum "A- " credit rating set by Standard & Poor's or equivalent,

— having reinsurance coverage from a reputable reinsurance company which has a minimum credit rating of "A-" fixed by Standard and Poor's or equivalent, if the amount of insurance for the particular subject of public procurement is greater than the amount of the maximum self-insured retention determined by the Reinsurance Program of the insurance company,

Услови за извршување на договорот:

Deposits and guarantees required:

1) Statement about the steadiness of the offer;

2) Performance guarantee (10 % of the total contract value). In order to sign the contract with the selected most favourable economic operator, the successful bidder shall be required to provide a contract performance bank guarantee at the amount of 10 % of the contract value, for each separate lot.

3) Statement for independent offer.
