Confirmation for registered business activity, confirmation for chemical trade issued by the Ministry of health of the Republic of North Macedonia – Medicines Agency, in accordance with Article 65, paragraph 2, of Law on Chemicals published in the Official Gazette of the Republic of Macedonia No.145/10.

— statement that no final court verdict has been pronounced in the last 5 years for a crime committed by Article 88 paragraph 1 of the PPL,

— confirmation that no bankruptcy procedure has been initiated by a competent authority,

— confirmation that no liquidation procedure has been initiated by a competent authority,

— confirmation of paid taxes, contributions and other public fees from the authority competent in the country where the economic operator has been registered,

— confirmation from the registry for penalties for…

… committed criminal acts of the entities stating that no minor penalty banning the participation in open call procedures, awarding of contracts for public procurement and contracts on public-private partnership has been issued,

… criminal acts of legal entities that no minor penalty has been issued, temporary or permanent ban on performing a particular business activity,

— confirmation that no penalty sanction has been issued – ban on carrying out a profession, business activity or duty, i.e. temporary ban on performing a particular business activity.

Техничка и професионална способност:

— with a list of major deliveries of goods or services performed in the last 3 years, with values, dates, buyers (contracting authorities or economic operators) by providing confirmation of deliveries made or services performed,

— with a description of the technical staff and technical authorities involved in the performance of the public service procurement, in particular those responsible for quality control, and in the case of public procurement work, those who will be involved in doing the work weather they are employed at economic operator or not,

— by specifying the supply chain management and monitoring system that the economic operator will be able to apply it when executing the contract,

— standards for quality systems,

— standards for environmental management.

Минимално ниво(а) на стандарди:

— list of minimum 2(two) WWTP polymer deliveries in the last 2 years, with quantities greater than 1 000 (thousand) kg,

— proof of at least 2 deliveries,

– contract or confirmation for satisfactory delivery,

— confirmation of satisfactory results of polymer application,

— certificate of trained personnel, engineer-technologist working for polymers,

– certificate for trained personnel, mechanical engineer with minimum work experience of 10 years in the field of water treatment dosage systems,

— manufacturer 's certificate for the subject of the supply that is offered,

— manufacturer's dealership agreement valid for at least the end of 2021 year,

— original manufacturer's catalog of the subject of procurement offered,

– safe lists (MSDS), in Macedonian,

— authorization issued and certified by the manufacturer,

— statement that the polymers will be shipped in original packaging by a manufacturer with a declaration issued by manufacturer, without the possibility of repackaging or rebranding,

— valid certificates of ISO 9001: 2015 quality system or equivalent,

— valid certificate for standard protection for environment ISO 14001: 2015 or equivalent.
