Blood bags by letting the use of equipment for blood processing and blood components under technical specification. It includes:
1) System of fourfold blood bags with anticoagulant CPD and preservative SAG-Manitol with volume of 450 ml (+/- 5 %) with configuration of the system Top & Bottom.
2) System of fourfold blood bags with anticoagulant CPD and preservative SAG-Manitol with volume of 450 ml (+/- 5 %) with configuration of the system Top & Top.
With the offered bags it should be used the following equipment for blood processing and blood components for a period of 2 years:
1) The appliance for automatic separation of the blood components.
2) An appliance for automatic sticking of guts.
3) The economic operator is required to provide, without charge, an appropriate quantity of accessories:
— rings,
— pliers.

2) An appliance for automatic sticking of guts.

3) The economic operator is required to provide, without charge, an appropriate quantity of accessories:

1) The appliance for automatic separation of the blood components - 12 pieces.

2) An appliance for automatic sticking of guts - 6 pieces.

— pliers 40 pieces.

— decision for wholesale of medical devices in accordance with the Law on Medicines and Medical Devices;

— statement of the economic operator that the economic operator or the person who is a member of the management or of the supervisory board of such economic operator or who is authorized for representation or decision-making or supervision thereof has not been pronounced in the last five years with a valid court verdict for committed criminal act including elements of the criminal acts participation in criminal association, corruption, evasion of taxes and contribution, terrorism or of fences related to terrorist activities, money laundering and terrorism financing and child labor and trafficking in humans, as prescribed with the Criminal Code;

— confirmation of paid taxes, contributions and other public dues issued by a competent authority in the country where the economic operator is registered;

— confirmation that the economic operator is not subject to an opened…

… bankruptcy procedure by a competent authority;

… liquidation procedure by a competent authority;

— confirmation from the Registry of penalties for committed criminal acts of the legal entities that the economic operator was not pronounced with a second penalty…

… prohibiting the participation in the public call procedures, the public procurement contract award procedures and the public private partnership procedures;

… for temporary prohibition for performing certain activity;

… for permanent prohibition for performing certain activity;

— confirmation that there is no legally effective judgment imposing a misdemeanor sanction –…

… prohibition to perform a profession, activity or duty imposed against the economic operator;

… temporary prohibition for performing certain activity imposed against the economic operator.

Техничка и професионална способност:

— the economic operator should provide a catalog or other documents with technical specifications of the offered goods (for the offered bags for blood and equipment) which will prove the requirements of the technical specification with clearly marked pages and marked each request from the specification;

— the economic operator is required to provide samples of the offered blood bags in original non-open packaging;

— the economic operator is obliged to train the medical personnel who will handle the equipment for blood processing and blood components that will be used for which they will issue an appropriate confirmation / certificate for training;

— the economic operator is obliged to provide service and maintenance of the equipment for blood processing and blood components that will be used for free of charge for a period of 2 (two) years.

Минимално ниво(а) на стандарди:

— catalog or other documents with technical specifications of the offered goods (for the offered blood and equipment bags) which will prove the requirements of the technical specification with clearly marked pages and marked each request from the specification;

— samples from the offered blood bags in original non-open packaging;

— a statement from the economic operator that will…
