— Statement that in the last 5 years no effective court verdict has been issued for committing an act of crime pursuant to Article 88 paragraph 1 of the Law on Public Procurement;

— confirmation stating that no bankruptcy procedure has been initiated by a competent body;

— confirmation stating that no liquidation procedure has been initiated by a competent body;

— certificate of paid taxes, benefits and other public duties issued by a competent body in the country where the economic operator is registered;

— confirmation from the Register of penalties for committed criminal offenses of legal entities proving that no secondary penalty —…

— with the educational and professional qualifications of the service provider or the contractor or his management staff, provided that they are not evaluated as the criterion for selecting the most favourable bid:-Copy of certificate for working with disk capacity devices issued by the manufacturer of offered equipment. Employment agreement or employment contract;

— certificates and reports issued by competent bodies for quality control with acknowledged competence, and that test the compliance of the products which was clearly set in the references of the specifications and standards: -Copy of document by the manufacturer MAF (Manufacturer Authorization Form).

• ISO 9001:2015 Certificate (Quality Management System) or equivalent, issued by accredited body, including number, date of issuance and current validity of the certificate;

• ISO 27000-1:2013 Certificate (Information security management) or equivalent, issued by accredited body, including number, date of issuance and current validity of the certificate;

• ISO/IEC 20000-1:2011 Certificate (IT Service management) or equivalent, issued by accredited body, including number, date of issuance and current validity of the certificate.
