— statement that in the past 5 years there was not any final judgment for penalty of Article 88 paragraph 1 of the PPL,

— confirmation for paid taxes, contributions and other public duties by a competent organ in the country where the economic operator was registered,
— certificate from the Registry of penalties for crimes committed by legal persons that…
… minor penalty ban on participation in public tender procedures, awarding contracts and agreements for public-private partnership has not been imposed on him,
… a minor penalty for temporary or permanent prohibition of a specific activity has not been imposed over him,
— confirmation that a final judgment — prohibition of professional activity or duty, i.e. temporary prohibition for performing professional activity has not been imposed over him.

The operator shall prove the meeting of the minimum conditions of the sub-point by submitting the following:
— for registered medical devices:
— decision for putting into market circulation of a medical device or temporary or permanent approval for parallel import, pursuant to the law on medicines and medical devices.
For unregistered medical devices:
— import license (approval for intervention import) pursuant to the law on medicines and medical devices, or document that proves that a procedure has been started for obtaining of approval for intervention import for the equipment subject to this procedure.

Минимално ниво(а) на стандарди:
— certificates and reports issued by competent organs for quality control with acknowledged competence, which test the convenience of products that was determined by the references to the specifications and standards.
For registered medical devices: for the offered goods, the economic operator shall provide approval for putting into market circulation of a medical device or temporary or permanent approval for parallel import, pursuant to the law on medicines and medical devices.
